People tend to lump every itchy rash under "sensitive skin," but hives and eczema are different problems needing different approaches. The most reliable way to tell them apart isn't what they look like — it's how long they stay and what they leave behind.

Ramai menganggap semua ruam gatal sebagai "kulit sensitif," tetapi biduri dan ekzema adalah masalah berbeza. Cara paling boleh dipercayai membezakannya bukan rupanya — tetapi berapa lama ia bertahan dan apa yang ditinggalkannya.

On the arm today, the thigh tomorrow; a single weal usually settles within 24 hours and the skin underneath looks entirely normal afterwards — no scaling, no thickening, no mark. That's because it's a transient leak from small vessels in the upper skin lifting the surface; once the swelling goes, nothing remains. Which is why photographing a flare is unusually useful here — by the time you reach the practice, it has often vanished.

The same patch, over and over, for days, weeks or months. Given time the skin thickens, roughens, scales and darkens, and scratching it open leads to weeping and crusting. One line to remember: moving around and clearing clean is urticaria; staying put and coarsening the skin is eczema. If yours looks like both, or if a weal fades leaving a bruise or pigment behind, it isn't typical urticaria and needs to be looked at properly.

The great majority of hives are just itchy and maddening, but a small proportion turn dangerous. We've put this section up front because it matters more than everything else in this article.

Any one of the following means going to a doctor or A&E immediately — do not come to a TCM practice first, and do not wait it out at home: swelling of the lips, eyelids, tongue or throat; a tight throat, hoarse voice, or trouble swallowing; difficulty breathing, wheeze, or chest tightness; dizziness, cold sweat, racing heart, or feeling about to faint; weals together with severe abdominal pain or vomiting. These can indicate angioedema or anaphylaxis — life-threatening, and capable of worsening very quickly. If you've ever been prescribed an adrenaline pen (EpiPen) for allergy, carry it and use it as instructed. These next ones aren't emergencies, but they mean it isn't typical urticaria and warrant medical investigation: weals with fever or joint pain; a single weal lasting beyond 24 to 48 hours; fading that leaves bruising, pigment or scaling. TCM care and medical investigation are never in conflict — the dangerous possibilities get excluded first, and what remains, the chronic recurring kind, is where TCM has something to offer.

临床上常用六周作为分界:六周内缓解的叫急性荨麻疹,多半找得到明确诱因——某一次海鲜、某个药物、一场感冒或感染。反复发作超过六周,就叫慢性荨麻疹,而这正是很多人最挫折的阶段:验了一轮过敏原,报告全部正常,医师说「找不到原因」,自己却每天在痒。

Clinically, six weeks is the usual dividing line. Settling within six weeks is acute urticaria, and there's often a clear trigger — one seafood meal, a medication, a cold or infection. Recurring beyond six weeks is chronic urticaria, and that's where the frustration sets in: a full allergy panel comes back clean, you're told no cause can be found, and yet you're itching every day.

Secara klinikal, enam minggu ialah garis pembahagi biasa. Reda dalam enam minggu ialah biduri akut, dan selalunya ada pencetus jelas. Berulang melebihi enam minggu ialah biduri kronik — dan di situlah kekecewaan bermula: ujian alahan bersih, tiada punca ditemui, tetapi anda gatal setiap hari.

相当比例的慢性荨麻疹患者,根本找不到单一的外在过敏原——这不是检查做得不够,而是这个病本来就常常这样。真正在变的,是身体被触发的门槛降低了:本来要很强的刺激才会起反应,现在压力大、睡不够、一场小感冒、天气一变、衣服勒紧一点、洗个热水澡,都足以让它冒出来。所以一直追问「我到底对什么过敏」,往往不如换个问题:为什么我的身体现在这么容易被触发?这两个问题的差别很实际——前者的答案是「避开某样东西」,后者的答案是「把反应的阈值调回去」。中医处理荨麻疹,走的是后面这条路:不是替你找出那只虾,而是让身体没那么容易一碰就炸。

A substantial share of people with chronic urticaria have no single external allergen to find — not because the testing was inadequate, but because that's simply how this condition often behaves. What has changed is the threshold at which the body reacts: what once needed a strong provocation now takes only stress, a short night, a mild infection, a shift in weather, tight clothing, or a hot shower. So rather than pressing on with "what am I allergic to," the more useful question is: why is my body currently so easy to set off? The difference is practical — the first question's answer is "avoid a thing," the second's is "raise the threshold back up." TCM works along the second road: not finding the prawn for you, but making the system less quick to fire.

Sebahagian besar pesakit biduri kronik memang tiada alergen luaran tunggal untuk ditemui — bukan kerana ujian tak mencukupi, tetapi begitulah keadaan penyakit ini. Yang berubah ialah ambang tindak balas badan: apa yang dahulu memerlukan rangsangan kuat kini hanya perlukan tekanan, kurang tidur, jangkitan ringan, perubahan cuaca, pakaian ketat, atau mandi air panas. The classical TCM name for urticaria translates as "hidden rash," and wind is its defining quality — fast onset, migrating location, sudden appearance, all of which is what TCM means by wind. But the conditions behind that wind differ, and so does the direction of treatment. It's why two people both breaking out in weals can be given opposite approaches. Comes on with air-conditioning, cold water, a cool shower or a drop in temperature; the weals are pale or lightly pink, and warmth actually eases them. People with this pattern often say "my arms start itching the moment I walk into an air-conditioned room." 和上面正好相反:洗热水澡、运动流汗、天气闷热、情绪一激动就冒,风团色红、摸起来热、痒得比较厉害,冷敷会舒服。这一型如果误用温补的方法(例如自行喝姜茶、吃补),常常更糟——「热」的概念可以看上火/热气篇

The exact opposite: a hot shower, exercise and sweat, muggy weather or a surge of emotion brings it up; the weals are red, warm to touch and more intensely itchy, and a cold compress helps. Especially common given Malaysia's climate and diet: red weals with a moist quality, an itch that feels sticky and clinging, often alongside a pasty or bitter mouth, heaviness in the body, loose sticky stools and oily skin. 病程拖很久、反复几个月甚至几年的人常见这一型:皮肤偏干、风团颜色偏淡、多在傍晚或夜里发作,常合并疲倦、面色偏白、睡不好、女性经量偏少。这一型不是「清一清就好」,方向反而是养血润燥——这也是为什么慢性荨麻疹不适合一直用清热的方法硬清,清久了反而更干更痒。

Typical of long-standing cases running for months or years: drier skin, paler weals, flares in the evening or at night, often with fatigue, a pale complexion, poor sleep, and lighter periods in women. This one doesn't respond to "clearing" — the direction is nourishing blood and moistening dryness. It's precisely why chronic urticaria shouldn't be met with endless cooling measures, which over time leave the skin drier and itchier.

Tipikal kes berlarutan berbulan atau bertahun: kulit lebih kering, bengkak lebih pucat, serangan pada waktu petang atau malam, sering dengan keletihan dan tidur terganggu. Corak ini tidak bertindak balas kepada kaedah "menyejukkan" — arahnya memelihara darah dan melembapkan kekeringan.

要提醒的是:这四型在真实的人身上常常混着出现,也会随着季节、压力、月经周期变来变去。所以对照上面的描述认出自己大概是哪一型,是有帮助的第一步,但真正的分型要由医师看舌、把脉、问诊之后决定——尤其风寒和风热这两型方向相反,认错了,方法就用反了。

A caveat: in real people these four commonly overlap, and they shift with the season, stress levels and menstrual cycle. Recognising roughly which one sounds like you is a useful first step, but the actual differentiation is made by a practitioner after looking at your tongue, feeling your pulse and asking the questions — particularly because wind-cold and wind-heat point in opposite directions, and getting that backwards means doing the opposite of what helps.

先说清楚定位:急性发作、尤其伴随上面那些急症信号的,请走西医;中医擅长的是反复超过六周、查不出明确原因、抗组织胺一停就回来的那一段。医师会先四诊评估分型,用科学中药为主,视情况配合针灸等外治法,目标是把身体过度反应的状态调回来。

To be clear about scope: acute attacks — especially with any of the emergency signs above — belong with medical care. Where TCM contributes is the stretch beyond six weeks, with no identifiable cause, where the hives return as soon as antihistamines stop. The practitioner differentiates the pattern through the four examinations, works mainly with concentrated herbal granules, and adds acupuncture or other external therapies where appropriate — the aim being to settle a system that is over-reacting.

Untuk menjelaskan skop: serangan akut — terutamanya dengan tanda kecemasan di atas — adalah untuk rawatan perubatan. TCM menyumbang pada tempoh melebihi enam minggu, tanpa punca dikenal pasti. Pengamal membezakan corak melalui empat pemeriksaan dan bekerja terutamanya dengan ubat herba tertumpu.

合理的期待长什么样,我们讲得直接一点。慢性荨麻疹是个需要时间的问题,任何人跟你保证「几帖就让它永远不再发」,都不要信。我们看的是三个维度一起变:发作的程度变轻(风团变小、变少、痒得没那么厉害,不再痒到抓破或睡不着)、发作的次数变少(从天天发变成几天一次,再拉长)、每次持续的时间缩短(以前一发一整晚,现在一两个小时就退)。这三项通常不是同时改善,很多人是先觉得「痒得没那么难受了」,次数才慢慢下来。还有一件很重要的事:如果你正在吃抗组织胺,不要因为开始看中医就自己停掉。两者可以并行,减药、停药的时机由医师判断——突然停药常常换来一次更猛的反弹,那不值得。

Let's be direct about what a reasonable expectation looks like. Chronic urticaria takes time, and anyone promising to clear it for good in a few doses shouldn't be believed. We watch three dimensions move together: flares become milder (smaller, fewer weals, an itch that no longer has you scratching raw or awake at night), less frequent (from daily to every few days, then longer), and shorter each time (what used to last all night now settles in an hour or two). These rarely improve in step — most people notice the itch becoming bearable first, with frequency following later. And one thing that matters: if you're on antihistamines, don't stop them yourself just because you've started TCM. The two run alongside each other, and when to reduce or stop is a decision for your doctor — an abrupt stop often buys you a fiercer rebound, which isn't worth it.

  • 别抓,改成轻拍或冷敷。搔抓会让风团沿着抓痕扩大、痒得更久,形成越抓越痒、越痒越抓的循环。想缓解就轻拍、用凉毛巾冷敷,或穿件宽松衣物隔一层。指甲剪短,晚上痒到睡不着的话,先冷敷再躺下会好过一点。 Don't scratch — pat or cool it instead. Scratching makes weals spread along the scratch line and prolongs the itch, feeding the cycle. Pat the area, use a cool damp towel, or put a loose layer of clothing between hand and skin. Keep nails short, and if the itch keeps you awake, cool the area before lying down. Jangan garu — tepuk atau sejukkan. Menggaru menyebabkan bengkak merebak mengikut garis garuan dan memanjangkan gatal. Tepuk kawasan itu, guna tuala lembap sejuk, atau letak lapisan pakaian longgar. Potong kuku pendek.
  • 洗澡用温水,别用很烫的水冲。很多人痒起来会想用热水冲,因为烫的当下确实压得住痒——但热会让多数人的风团更明显,冲完那阵舒服过去,反弹常常更厉害。水温温的就好,冲完轻轻拍干,别用力搓。同样道理,发作期间先别泡热水澡、别蒸桑拿。 Shower warm, not scalding. The instinct when itching is to blast it with hot water, because the heat does override the itch in the moment — but heat makes weals more prominent for most people, and once that brief relief passes the rebound is often worse. Keep the water lukewarm, pat dry gently rather than rubbing, and skip hot baths and saunas while you're flaring. Mandi suam, bukan panas membakar.   • 做一份发作记录,比再验一次过敏原有用。每次发作记下:时间、前几小时吃了什么、当时在做什么(刚运动完?刚进冷气房?刚洗完热水澡?)、天气、压力和睡眠状况、女性再记月经周期。两三周下来,个人的规律常常自己浮出来——而这份规律是任何检测报告都给不了你的。看诊时把它带来(或拍照存手机),医师能据此判断得准很多。发作时也请拍照,因为到诊所时通常已经退光了。 Keep a flare diary — it's worth more than another allergy panel. For each flare note the time, what you ate in the preceding hours, what you were doing (just exercised? just walked into air-conditioning? just had a hot shower?), the weather, your stress and sleep, and for women the point in your cycle. Over two or three weeks a personal pattern usually surfaces — and that pattern is something no test report can give you. Bring it to your appointment, and photograph the flares, since they've usually vanished by the time you arrive. Simpan diari serangan — ia lebih bernilai daripada satu lagi ujian alahan.   • 别在没有证据的情况下大范围忌口。很多人一发风疹块就把海鲜、鸡蛋、牛奶、豆类全戒掉,戒到最后营养不均衡、身体更虚,风团照样来。已知明确会诱发的食物当然要避开,但没有证据支持的全面忌口弊大于利。真正常见的诱因反而常是「量」和「状态」的组合——同一份海鲜,睡饱时吃没事,熬夜加喝酒之后吃就发。这一点在榴莲那篇也讲过:问题往往不在那样东西本身。 Don't cut out whole food groups without evidence. Plenty of people drop seafood, eggs, dairy and legumes at the first weal, and end up undernourished and more depleted with the hives still coming. Avoid what demonstrably sets you off, certainly — but sweeping elimination without evidence does more harm than good. The commonest real trigger is a combination of quantity and state: the same seafood is fine when you're well rested and sets you off after a late night and a few drinks. We made the same point in our durian article — the problem often isn't the food itself. Jangan potong seluruh kumpulan makanan tanpa bukti. Ramai berhenti makan makanan laut, telur, susu dan kekacang serentak, lalu kekurangan nutrisi sementara biduri tetap datang.
