「来,舌头伸出来看看。」几乎每次看中医都有这一步。你可能觉得就是随便看看,其实医师这几秒钟里读到的信息,不比把脉少。舌头是身体少数可以直接「看见」的内部器官——它的颜色、那层苔、形状,都在悄悄反映你身体里正在发生的事。这篇教你看懂医师在看什么,顺便,你也可以对着镜子看看自己的舌头。
"Let me see your tongue." Almost every TCM visit has this step. It may look like a casual glance, but what the practitioner reads in those few seconds is no less than the pulse. The tongue is one of the few internal organs you can see directly — its colour, that coating, its shape all quietly reflect what's happening inside. Here's what the practitioner is reading — and you can check your own tongue in the mirror too.
"Jom, julurkan lidah." Hampir setiap lawatan TCM ada langkah ini. Ia mungkin nampak seperti pandangan sambil lewa, tetapi apa yang pengamal baca dalam beberapa saat itu tidak kurang dari nadi. Lidah antara organ dalaman yang boleh anda lihat terus.

舌头为什么能反映身体?因为它黏膜薄、血管和神经丰富,身体内部的状态变化,很快会在舌头上显现出来。医师看舌,主要看三样,各对应不同的信息:
Why does the tongue reflect the body? Its mucous membrane is thin and rich in blood vessels and nerves, so internal changes show up on it quickly. A practitioner reads three things, each carrying different information:
Mengapa lidah mencerminkan badan? Membran mukusnya nipis dan kaya saluran darah, jadi perubahan dalaman muncul dengan cepat. Pengamal membaca tiga perkara:
反映气血和寒热虚实。淡白,多提示气血不足或偏虚寒;偏红,多提示有热;红得深或绛红,热更重;紫暗或有瘀点,多提示血瘀、气血运行不畅。
Reflects qi, blood, and cold/heat, deficiency/excess. Pale often means insufficient qi-blood or cold; red means heat; deep or crimson red, stronger heat; purple or with spots often means blood stasis and poor circulation.
Mencerminkan qi, darah, sejuk/panas. Pucat = qi-darah kurang; merah = panas; ungu atau bertompok = stasis darah.
反映脾胃运化和湿、热、痰的状态。正常应是薄白、透得出舌质。变厚变腻,多提示体内有湿或痰;苔色偏黄,多提示有热;苔很少甚至没有(光剥),多提示阴伤。
Reflects spleen-stomach function and the state of damp, heat, and phlegm. Normally thin, white, showing the body through. Thick and greasy suggests damp or phlegm; yellow suggests heat; scanty or peeled suggests damaged yin.
Mencerminkan fungsi limpa-perut dan lembap, panas, kahak. Biasanya nipis putih. Tebal berminyak = lembap; kuning = panas; sedikit/terkelupas = yin rosak.
第三样是舌形:舌体胖大、边缘有牙齿压出的齿痕,多和气虚或湿气重有关;舌面有裂纹,多提示阴液不足;舌体瘦薄,多提示气血或阴液亏虚。三样合起来,医师就能拼出不少信息。
The third is shape: a swollen tongue with tooth marks along the edges often relates to qi deficiency or dampness; cracks on the surface often point to insufficient yin; a thin tongue often means depleted qi-blood or yin. Combined, the three give the practitioner a lot to work with.
Ketiga ialah bentuk: lidah bengkak dengan tanda gigi = kekurangan qi atau lembap; retakan = yin tidak cukup; lidah nipis = qi-darah kurang.

除了看整体,医师还会看舌头不同区域。像上面那张图:舌尖对应心肺,舌中对应脾胃,舌两边对应肝胆,舌根对应肾。所以同样是「有点异常」,出现在舌尖还是舌根,指向可能完全不同。
Beyond the whole, the practitioner reads different regions. As in the diagram: the tip maps to heart and lung, the centre to spleen and stomach, the sides to liver and gallbladder, the root to the kidney. So the "same" abnormality points to something quite different depending on whether it's at the tip or the root.
Selain keseluruhan, pengamal membaca kawasan berbeza. Hujung = jantung-paru, tengah = limpa-perut, tepi = hati-hempedu, pangkal = buah pinggang.
下次刷牙时不妨看看自己的舌头:如果舌苔白厚、舌边有明显齿痕,可能提示湿气偏重(可以看 「湿气重」到底是什么);如果舌红苔黄,可能偏热(对照 「上火」是真的吗)。这些能帮你大致感受方向,但自己看只是参考——舌象常常几个信号叠加,真正辨证还是要交给医师。
Next time you brush your teeth, take a look: a thick white coating with clear tooth marks may suggest dampness (see What Is "Dampness"); a red tongue with yellow coating may lean toward heat (see Is "Heatiness" Real?). These give you a rough sense of direction, but self-observation is only a reference — tongue signs often layer together, and real pattern differentiation is for the practitioner.

舌象容易被外在因素干扰,看之前(或看诊当天)注意避开这几件事,否则很容易误判:
Tongue signs are easily thrown off by outside factors. Before checking (or on the day of your visit), avoid these, or it's easy to misread:
Tanda lidah mudah terganggu faktor luar. Sebelum menyemak, elak perkara ini:
- 别吃会染色的东西。咖啡、kopi、奶茶、糖果、某些饮料和药物会把舌苔染色,橙黄、深褐的「染苔」很容易被误当成病理表现。看诊前尽量别刚喝完 kopi。Avoid staining foods. Coffee, kopi, milk tea, sweets and some drinks or medicines stain the coating; an orange or dark-brown "stained coating" is easily mistaken for a pathological sign. Try not to have just finished a kopi before your visit.Elak makanan mewarna. Kopi, teh susu, gula-gula mewarnakan salut; "salut terwarna" mudah disalah anggap. Cuba jangan baru habis minum kopi.
- 别刮或刷舌苔。很多人习惯把舌苔刮干净,但看诊前刮掉,医师就看不到真实的苔了。让它自然呈现最准。Don't scrape or brush off the coating. Many habitually scrape the tongue clean, but removing it before a visit hides the real coating. Let it show naturally for the truest read.Jangan kikis salut. Ramai biasa mengikis lidah, tetapi membuangnya sebelum lawatan menyembunyikan salut sebenar.
- 光线要自然,别太暗或彩色灯。彩色或昏暗灯光会让舌色失真。最好在早上起床后、自然光下、舌头自然伸出放松地看。Use natural light, not dim or coloured light. Coloured or dim lighting distorts the tongue colour. Best viewed in the morning, in natural light, with the tongue relaxed and naturally extended.Guna cahaya semula jadi. Cahaya berwarna atau malap memesongkan warna lidah. Terbaik pada waktu pagi, cahaya semula jadi.

Because the tongue changes relatively quickly, re-observing it at follow-ups is an intuitive way to judge whether treatment is on track.
