A practitioner rests three fingers on your wrist, goes quiet for a few seconds, then describes a string of things about your body — and many first-timers watch this with equal parts curiosity and doubt: can a pulse really tell all that? 脉,是心脏搏动通过血管传到手腕的震动。中医认为,这个搏动同时也反映了全身气血运行的状态——所以医师读的不是「心跳几下」这么简单,而是脉象里藏着的一整套信息。就像上面那张图,医师的手指同时在感受四个维度:
The pulse is the heartbeat transmitted through the vessels to the wrist. TCM holds that this beat also reflects the state of qi and blood moving through the whole body — so the practitioner isn't just counting beats, but reading a whole set of information in the pulse's quality. As in the diagram above, the fingers sense four dimensions at once:
Nadi ialah degupan jantung yang dihantar melalui saluran ke pergelangan. TCM berpendapat degupan ini juga mencerminkan keadaan qi dan darah seluruh badan. Jari merasai empat dimensi sekaligus:
脉跳得快,常和「热」有关;跳得慢,常和「寒」有关。这不是单看数字,而是结合整体来判断——同样偏快,实热和虚热的脉又不一样。
A faster pulse often relates to "heat", a slower one to "cold". It's not the number alone — read in context, the same fast rate feels different in excess-heat versus deficiency-heat.
Nadi lebih laju sering berkait "panas", lebih perlahan dengan "sejuk". Bukan nombor semata — dibaca dalam konteks.
轻轻一按就摸到(浮),常提示问题偏「表」,如刚受外感;要重按才摸到(沉),常提示问题偏「里」,在脏腑更深处。
Felt with a light touch (floating) often points to a "surface" issue, like a fresh external cold; only felt with firm pressure (sinking) points to a deeper, "interior" issue in the organs.
Terasa dengan sentuhan ringan (terapung) menunjuk isu "permukaan"; hanya terasa dengan tekanan kuat (tenggelam) menunjuk isu lebih dalam.
脉搏有力,多属「实」;脉搏细弱无力,多属「虚」。这直接帮医师判断,你的问题是身体某处太过(该清),还是不足(该补)。
A forceful pulse tends toward "excess"; a thin, weak pulse toward "deficiency". This directly helps the practitioner judge whether your issue is something in excess (to clear) or lacking (to tonify).
Nadi kuat cenderung "lebihan"; nadi nipis lemah cenderung "kekurangan". Ini membantu menilai sama ada perlu dibersih atau dikuatkan.
脉走得顺滑(滑脉),还是涩滞不畅(涩脉),提示气血是通还是堵。比如血瘀的人,脉常偏涩;痰湿重的人,脉常偏滑。
Whether the pulse runs smooth (slippery) or hesitant (choppy) points to whether qi and blood flow freely or are stuck. Blood stasis often gives a choppy pulse; heavy phlegm-damp, a slippery one.
Sama ada nadi lancar (licin) atau tersekat (kasar) menunjuk aliran qi dan darah. Stasis darah sering nadi kasar; kahak-lembap, nadi licin.

你可能注意到,医师把脉时手指会分三个位置轻轻挪动,两只手都摸。这三个位置从手腕往手肘方向依次叫寸、关、尺,它们分别对应不同的脏腑系统,左右手对应的还不一样。
You may have noticed the fingers shift across three spots, on both wrists. From the wrist toward the elbow, these are called cun, guan and chi — each mapping to different organ systems, and the left and right wrists map differently too.
Anda mungkin perasan jari beralih merentasi tiga tempat, pada kedua pergelangan. Dari pergelangan ke siku, ia dipanggil cun, guan dan chi — masing-masing memetakan sistem organ berbeza.
正因为寸关尺对应不同系统,医师通过比较不同部位脉象的差异,可以大致判断问题偏向哪个脏腑。比如尺部(对应肾)脉沉弱,可能提示肾气不足;关部(对应肝)脉弦紧,可能提示肝气不舒。这就是为什么把脉不是随便搭一下,而是两手、分三部、逐一细读。
Because cun, guan and chi map to different systems, comparing the differences between positions helps the practitioner localise which organ system leans off-balance. A sinking, weak chi (kidney) may suggest insufficient kidney qi; a taut guan (liver) may suggest constrained liver qi. This is why pulse-taking isn't a casual touch, but a careful read across both hands and three positions.

把脉常被神化,好像医师一搭手就能算出你所有过去未来。要澄清两点:第一,脉诊读的是身体当下的状态,是一项需要长期训练的诊断技能,不是玄学;第二,它也不是万能的,不能单凭一个脉象看出所有的病。
Pulse diagnosis often gets mythologised, as if one touch reveals your entire past and future. Two clarifications: first, it reads the body's current state — a diagnostic skill requiring years of training, not mysticism; second, it isn't all-powerful, and a single pulse can't reveal every disease.
Diagnosis nadi sering dimitoskan, seolah satu sentuhan mendedahkan seluruh masa lalu anda. Dua penjelasan: pertama, ia membaca keadaan semasa badan — kemahiran memerlukan latihan bertahun; kedua, ia bukan serba boleh.
脉诊不能单打独斗。中医讲「四诊合参」——把脉(切)只是望、闻、问、切四诊中的一环,必须和看气色舌象、问生活细节合起来,才能准确辨证。所以医师不会只凭脉就下结论。至于常听到的「把脉能测怀孕、看胎儿性别」:有经验的医师可能从滑脉察觉妊娠的迹象,但这不是确诊工具,准确度远不如现代产科检查——想确认还是要靠验孕和产检。把脉的价值是真实的,但不该被神化。
The pulse doesn't work alone. TCM relies on "four examinations combined" — pulse-taking is just one of observation, listening, questioning and palpation, and must be combined with complexion, tongue and life details to differentiate a pattern accurately. So a practitioner won't conclude from the pulse alone. As for the familiar "the pulse can detect pregnancy or a baby's sex": an experienced practitioner may sense pregnancy from a slippery pulse, but it's not a diagnostic tool and is far less accurate than modern obstetric checks — confirm with a pregnancy test and prenatal care. The pulse's value is real, but shouldn't be mythologised.
Nadi tidak berfungsi sendirian. TCM bergantung pada "empat pemeriksaan digabung" — mengambil nadi hanyalah satu daripada memerhati, mendengar, menyoal dan meraba.
