You spot it in your child's graduation photo; or a bra strap keeps slipping off one side; or a parent reads too much online and grows more anxious by the minute — "is this scoliosis? Is it serious? Will she need surgery one day?" It's one of the questions parents most often bring into our centre. This piece answers it directly: an uneven shoulder is not the same as scoliosis. How to tell them apart, how to screen at home, when you must get an X-ray and see an orthopaedic specialist promptly, and what TCM can — and cannot — do here. This is the most important sentence in the article, so it goes first: they can look identical and be fundamentally different. In clinic, the most common cause of "one shoulder higher" isn't scoliosis at all — it's postural muscle imbalance: a bag always on the same shoulder, a habitual lean, always resting on one side, a dominant arm doing too much. Over time the muscle tension on the two sides diverges and the outline goes crooked. The tell: ask them to stand tall and square the shoulders, and the difference visibly shrinks — because the bone hasn't deformed; muscle is pulling it out of line.

A true structural scoliosis is different. The spine itself curves sideways, and — critically — the vertebrae rotate. That rotation is the dividing line between structural and postural. Because the vertebrae turn, the attached ribs turn with them, so when the person bends forward one side of the back rises visibly into what's known as a rib hump. This kind of curve does not disappear because you stand tall, pull your chest up, or try hard to square yourself — and "sit up straighter" won't fix it. Confirming it requires an X-ray to measure the Cobb angle — the orthopaedic standard for diagnosing scoliosis. Nobody can hand you that diagnosis by eyeballing a photo, ourselves included.

Because the two are handled in completely different directions, and going the wrong way costs you the most valuable thing — time. Postural: release the over-tight side, strengthen the weak side, change the daily habit creating it; this generally has decent room to improve. Structural: the priority is monitoring — especially in a child still growing, because a curve's rate of progression is tied directly to skeletal maturity, and the growth spurt is that window. Treat a rapidly progressing structural curve as "bad posture," rubbing it and telling the child to stand straight, and you may miss the point at which something should have been done. So our position is plain: tell them apart first, then decide what to do.

Crouch down so your eye line is roughly level with their back, and look along it from behind. Check whether the two sides — especially the rib area and the lower back — are level with each other. The point isn't whether the spine looks straight; it's whether the two sides are the same height.

Level and smooth on both sides → structural scoliosis is less likely, and a postural issue is more probable. One side clearly raised (a rib hump) → that's a significant clue for structural scoliosis, and imaging should be arranged promptly — don't wait, and don't go for massage or manipulation first. If in doubt, treat it as a yes and get it checked; that's the solid move.

Go straight to an orthopaedic specialist — don't come for manual therapy first — if: the forward bend test shows a clear rib hump; a rapidly growing adolescent (roughly 10–15) whose curve has visibly worsened over a few months; there's leg numbness, weakness, unsteady walking, or any change in bladder or bowel control; there's severe pain, pain waking them at night, or pain that doesn't ease with rest; or the curve comes with large skin patches, foot deformity, or other anomalies. These are red lines — this isn't us turning business away; these situations need imaging and a specialist's judgement, and starting with hands-on treatment won't help and may delay what matters. The growth spurt is when curves progress fastest. Finding it early and monitoring it beats everything else.

The "can't" comes first, because the market is far too vague about it: an established structural scoliosis cannot have its bony structure straightened by TCM. Tuina, bone-setting, acupuncture — none of them can undo vertebrae that have already rotated and deformed. If anyone tells you they'll "straighten the curve in a few sessions" or "correct it without a brace," raise your guard. We'd rather be clear with you now than have you spend the money and the time and end up disappointed.

To stay balanced, the body compensates elsewhere — commonly neck and shoulder ache, low back pain, a tilted pelvis, a leg-length sensation, and extra load on one knee or ankle. Those compensations are themselves treatable, and addressing them makes a real difference to how the person feels, even when the curve itself hasn't changed.

So if you bring your child in, we'll start with a postural assessment and the forward bend test, and settle that first gate — postural, or possibly structural. If we have structural concerns, we'll tell you plainly to get an X-ray and see an orthopaedic specialist — we're willing to say that, because it's the right order for your child. Once the diagnosis is clear, if the orthopaedic side is monitoring or bracing, we can work within that framework on the muscle tension, the pain, and the compensation, and make that stretch of road easier.

The road between the two: screen, keep records, and get an X-ray if there's any doubt. Confirmed postural, you can exhale and start on the habits. Confirmed structural, you've begun monitoring inside the window that matters most. Not knowing is the most draining state of all — just go and find out.
