孩子反复感冒、常年鼻塞流涕,西医多归为「反复呼吸道感染」和「过敏性鼻炎」。中医看的是背后的体质:多数是肺脾气虚、卫外不固。「肺」主管呼吸和体表的防御(中医叫「卫气」,像身体的守门员),肺气不足,风寒、过敏原一来就守不住,鼻子首当其冲;「脾」主管消化吸收、是生成正气的原料工厂,小孩「脾常不足」,若再加上偏食、贪凉、甜食冷饮多,脾胃更虚,正气的原料跟不上,于是形成「越虚越容易病、越病越虚」的循环。所以中医不把重点放在「这次感冒怎么好得快」,而放在不发作的时候,帮孩子把肺脾的底子补起来。
A child's recurrent colds and year-round blocked, runny nose are usually labelled "recurrent respiratory infections" and "allergic rhinitis" in Western medicine. TCM looks at the constitution behind them: most often lung-and-spleen qi deficiency with weak protective (wei) qi. The "lung" governs breathing and the body's surface defence (the wei qi, like a goalkeeper); when lung qi is low, wind-cold and allergens break through, and the nose is first hit. The "spleen" governs digestion and is the factory that makes the qi; children are "constitutionally spleen-weak," and picky eating, cold drinks and sweets weaken it further, so the raw material for qi runs short — creating a cycle of the weaker they are the more they fall ill, and the more they fall ill the weaker they get. So TCM's focus isn't "how to clear this cold fast" but building up the lung and spleen between illnesses.

TCM care suits the recurrent, lingering, slow-to-recover chronic picture; but when a child falls ill suddenly or severely, safety comes first. With any of the following, see a doctor first — with medicine or emergency care if needed: a high fever that won't settle or keeps recurring, or a listless child with fever; fast or laboured breathing, wheezing, or blue lips; a floppy, hard-to-wake, unusually drowsy or very irritable child; refusing to drink, passing little urine, no tears when crying (dehydration); severe headache with vomiting, or a stiff neck; ear pain or discharge, or a cough with chest pain or one that lingers; any fever in a baby under three months. TCM care never conflicts with seeing a doctor — we build the "baseline for staying well"; acute illness still needs medical care.

The deepest layer. Children are "constitutionally lung- and spleen-weak" to begin with; if the constitution is frail, or with prematurity or poor feeding, the protective (wei) qi guards the gate less well. Common signs: sweats easily, tires quickly, a pale or sallow face, so-so appetite, stools that swing between dry and loose. Qi is the baseline for staying well; a thin baseline means wind-cold and allergens get in easily — the very layer TCM most wants to build back up.

Malaysian children face a particular environmental challenge: hot outside, then strong air-conditioning the moment they enter a mall, classroom or car — hot-cold swings all day that a child's delicate airway hates; aircon blowing on them in sleep, kicking off the blanket, and a blocked, sneezy nose by morning. Add year-round damp with plenty of dust mites and mould — common triggers for rhinitis. These aren't "constitution" but adjustable external factors — the "what parents can do" section gets specific.

The link parents most often overlook, yet can most change. Iced water, soft drinks, ice cream and sweets are, in TCM's view, "raw-cold and cloying" and hardest on the spleen; a weak spleen means short raw material for qi and a tendency to breed "phlegm-damp" — which is behind many children's runny noses, phlegm and thick tongue coating. Picky, irregular eating and overeating to the point of food stagnation tire the spleen further. The spleen is the "acquired root"; much of a child's qi is, quite literally, eaten into being.
Kaitan yang ibu bapa paling kerap terlepas pandang, namun paling boleh diubah. Air ais, minuman bergas, aiskrim dan gula-gula, pada pandangan TCM, "sejuk-mentah dan melekit" dan paling membebankan limpa; limpa lemah bermakna bahan qi berkurang dan cenderung membiakkan "lembap-kahak" — di sebalik hingus dan kahak ramai anak. Many parents are puzzled: each doctor visit and course of medicine helps, so why does it flare again once stopped or the season turns? Because fever meds, antibiotics and antihistamines address "this episode's symptoms and infection" — use them when needed — but they don't change the child's prone baseline. With the baseline unbuilt, the next trigger brings another illness, so it grows more frequent, parents more anxious, the child more worn. This is exactly the gap TCM fills — not competing with Western medicine on acute care, but building the qi up between episodes so flares come less and milder. The two work together, not either-or.

Care for a child centres on "strengthening the qi and building the spleen": building the lung-and-spleen baseline between flares, while removing the triggers in daily life. Methods stay gentle and easy to accept — led by what a child will cooperate with, not centred on needling. Get it right and parents usually come to feel colds coming less often, milder each time, clearing faster, with better energy and appetite. This isn't a one- or two-session fix; it takes a little patience and partnership.

调体质的主力。医师会按孩子的体质、舌象辨证,用科学中药(浓缩颗粒)——补肺健脾、固护卫气(类似「玉屏风」的思路),有痰湿的兼顾化痰、有积食的帮着消导。颗粒按孩子体重开小剂量、冲水就能喝、味道比传统煎药淡很多,大多数孩子能接受。重点在「不发作时慢慢补底子」,不是感冒时才吃。所有用药都由 KKM 注册医师评估后开,别自己上网买给孩子乱吃。
The mainstay for shifting the constitution. By the child's pattern and tongue, the practitioner prescribes concentrated herbal granules — tonifying the lung, building the spleen, and firming the protective qi (along "Jade Screen" lines), resolving phlegm-damp or easing food stagnation where present. Granules are dosed to the child's weight, dissolve in water, and taste far milder than brewed decoctions, so most children accept them. The point is "slowly building the baseline between flares," not only during a cold. All prescribing is by a KKM-registered practitioner after assessment — don't buy herbs online for a child yourself.
Tunjang untuk mengubah perlembagaan. Mengikut corak dan lidah anak, pengamal memberi granul herba tumpat — menyuburkan paru, membina limpa, dan mengukuhkan qi pelindung. Acupoint plasters place warming herbs on relevant points on the back — nothing to swallow, nothing to needle; seasonal treatments like "San Fu Tie / San Jiu Tie" suit lung-spleen-weak, rhinitis-prone children well as a course. Gentle moxibustion warms and dispels cold, helping drive out "cold" and lift the yang qi. Children accept these external methods readily, making them a very practical part of paediatric care. Which points, and for how long, are set by the practitioner to the child's age and constitution.

Tampalan titik meletakkan herba hangat pada titik di belakang — tiada apa untuk ditelan, tiada jarum; rawatan bermusim seperti "San Fu Tie" sesuai untuk anak lemah paru-limpa. A child's qi is nurtured into being, so the practitioner gives parents specifics — how to eat (fewer raw-cold and cloying foods, mind the spleen), how to protect (aircon and temperature swings, warmth, routine, exercise and sunshine), and how to care during a flare. Clinic herbs and plasters "build the baseline"; home care "stops it leaking away again" — together, the constitution truly improves bit by bit. This is why paediatric care leans so much on parents — you're the one beside the child every day.

"Tonifying" also needs pattern diagnosis — forcing tonics on a child with food stagnation or internal heat backfires; online or grey-market "children's herbs / cooling teas / health powders" have unclear contents — have all herbs prescribed by a KKM-registered practitioner who has seen the child. ② Don't halve an adult's medicine for a child yourself — children's dosing and drug choice are specific. ③ It isn't either-or with Western medicine: acute infection, or a need to bring down fever or use antibiotics, is for a doctor — use them when needed; TCM handles the constitution between flares. Tell each practitioner what the child is taking. ④ During care, any of the "🚩" emergency signs above means seeing a doctor first.

- 保暖别贪凉,尤其管好冷气和温差。冷气别对着孩子直吹,室温别调太低;进出冷气房、商场、车里,给孩子备一件薄外套随手加;睡觉盖好肚子和背、别让孩子踢被子光着睡在冷气下;洗完澡先擦干、吹干头发再出房间,别让孩子湿着头吹风。「忽冷忽热」是马来西亚孩子最常见的诱因,管好这一条,往往就能少犯好几次。 Keep warm, not chilled — especially manage aircon and temperature swings. Don't let the aircon blow straight at your child, and don't set the room too cold; keep a light jacket handy for entering cold malls, rooms and cars; cover the tummy and back in sleep so they don't kick off the blanket and lie bare under the aircon; after a bath, dry off and dry the hair before leaving the room. Hot-cold swings are the commonest trigger for Malaysian children — manage this one and flares often drop noticeably. Kekal hangat, bukan sejuk — urus aircond dan pertukaran suhu. Jangan biar aircond meniup terus ke anak; sediakan jaket nipis untuk masuk pusat beli-belah dan kereta sejuk; tutup perut dan belakang semasa tidur; selepas mandi, keringkan rambut sebelum keluar bilik. Pertukaran panas-sejuk ialah pencetus paling biasa untuk anak Malaysia.
- 饮食顾脾胃:少冰饮甜食生冷,别一病就大补。尽量少给孩子冰水、汽水、雪糕、太多甜食和生冷食物——这些最伤脾胃、最容易生痰湿。吃饭定时、别撑太饱(积食也会诱发生病);生病刚好那几天,脾胃还虚,别急着「补一补」大鱼大肉,清淡好消化更养人。脾胃养好了,孩子的正气原料才跟得上。 Mind the spleen: fewer iced drinks, sweets and raw-cold foods, and don't over-tonify right after illness. Go easy on iced water, soft drinks, ice cream, lots of sweets and raw-cold foods — hardest on the spleen and quickest to breed phlegm-damp. Keep meals regular and not overfull (food stagnation can trigger illness too); in the days just after a cold, the spleen is still weak, so skip the rich "let's build him up" feasts — light, easy-to-digest food nourishes better. A well-tended spleen keeps the raw material for qi flowing. Jaga limpa: kurangkan minuman ais, gula-gula dan makanan sejuk-mentah, jangan menyubur berlebihan selepas sakit. Kurangkan air ais, minuman bergas, aiskrim dan makanan sejuk-mentah. Makan ikut waktu dan jangan terlalu kenyang; selepas selesema, limpa masih lemah, jadi elakkan jamuan berat — makanan ringan lebih memupuk.
- 记录发作规律,作息、运动、晒太阳一起顾。看诊前留意:孩子多久犯一次?什么时候最容易犯(换季?吹冷气后?吃了什么?)?每次拖几天好?有没有出汗多、易累、胃口差、大便情况?说得越具体,医师越好辨证。日常让孩子规律睡眠(睡眠养正气)、适度运动、适当晒太阳,把体质从底子练强——这些看起来普通,却是养正气最实在的方法。 Note the pattern, and mind sleep, exercise and sunshine. Before the visit, observe: how often does it flare? When is it worst (season change? after aircon? after certain foods?)? How many days to recover each time? Any heavy sweating, tiredness, poor appetite, or bowel changes? The more specific you are, the better the practitioner can read the pattern. Day to day, give the child regular sleep (sleep nourishes the qi), moderate exercise and some sunshine to strengthen the constitution from the ground up — ordinary-looking, but the most solid way to build the qi. Catat coraknya, dan jaga tidur, senaman dan cahaya matahari. Sebelum lawatan: berapa kerap ia berulang? Bila paling teruk (tukar musim? selepas aircond? selepas makanan tertentu?)?
