A cough that drags on for two or three weeks is something many people know well — the other cold symptoms cleared long ago, but the cough just won't leave. You take cough syrup, suck on herbal lozenges, it eases for a day, then comes back. The thing is, a cough isn't a disease in itself — it's the body signalling that something hasn't fully cleared. TCM doesn't just suppress the cough; it first works out which kind of cough you have.

中医有一句话:「五脏六腑皆令人咳,非独肺也。」意思是——咳嗽表面上在肺,根源却不一定只在肺。刚开始的急性咳嗽,多半是外感(吹风着凉、燥邪、热邪从口鼻而入);但一旦拖久,邪气由表入里,或者反复耗伤肺气、肺阴,就会变成缠绵难愈的久咳,越拖越难调。
There's a classic TCM line: "All the organs can cause a cough, not the lungs alone." On the surface the cough is in the lungs, but the root may lie elsewhere. An early, acute cough is usually from an external trigger — wind-cold, dryness, or heat entering through the nose and mouth. But once it lingers, the pathogen moves inward, or repeatedly drains the lung's qi and yin, turning into a stubborn chronic cough that gets harder to treat the longer it drags on.
Ada satu pepatah TCM: "Semua organ boleh menyebabkan batuk, bukan paru-paru sahaja." Di permukaan batuk itu di paru-paru, tetapi puncanya mungkin di tempat lain. Batuk akut awal biasanya dari pencetus luaran — angin-sejuk, kekeringan, atau haba. Tetapi apabila ia berlarutan, ia menjadi batuk kronik yang degil dan lebih sukar dirawat.
辨别咳嗽,第一步先看有没有痰。干咳无痰、或痰少而黏、喉咙干痒——偏燥咳或肺阴不足,重点在「润」;痰多容易咳出——则要再分白稀的寒痰和黄稠的热痰,重点在「化痰」。这两个方向几乎相反:燥咳去猛化痰会更干,寒咳去清热会更寒。分错方向,往往越止越难好。
Step one in sorting out a cough: is there phlegm? A dry cough with no phlegm, or scant sticky phlegm with a dry, itchy throat points to dryness or lung-yin deficiency — the focus is moistening. Copious phlegm that comes up easily needs further sorting into clear-thin (cold) or yellow-thick (heat) — the focus is resolving phlegm. These two directions are almost opposite: aggressively drying phlegm in a dry cough makes it drier; cooling a cold cough makes it colder. Get the direction wrong and it often gets worse.
Langkah pertama: ada kahak? Batuk kering tanpa kahak, atau kahak sedikit dan melekit dengan tekak kering dan gatal menunjukkan kekeringan — fokusnya melembapkan. Kahak banyak yang mudah keluar perlu diasingkan lagi kepada cair-sejuk atau kuning-panas. Kedua-dua arah hampir bertentangan — salah arah, ia menjadi lebih teruk.
在马六甲,还有几个很本地的诱因让咳嗽特别容易反复:长时间待在冷气房,空气干、温差大,容易咽干、干咳;烟霾季(haze)空气质量差,咽喉受刺激,痒咳、呛咳的人明显变多;加上长期潮湿闷热的气候,痰湿体质的人痰咳也很常见。所以同样是咳,本地人的诱因和体质,和课本上的未必一样。
In Melaka, a few very local triggers make coughs prone to recurring: long hours in air-conditioned rooms, where dry air and temperature swings bring on a dry, scratchy cough; the haze season, when poor air quality irritates the throat and itchy, choking coughs spike; and the constant humid heat, under which phlegm-damp constitutions produce phlegmy coughs. So even for the same cough, local triggers and constitutions don't always match the textbook.

Clinically, coughs fall into a few common patterns. Many people are a mix, or have dragged an acute cough into a chronic one — a practitioner's assessment sorts it out.
Secara klinikal, batuk terbahagi kepada beberapa corak biasa. Ramai orang adalah gabungan — penilaian pengamal mengasingkannya.
咳声重浊,痰白稀、量偏多,鼻塞流清涕,怕冷,受凉或吹到冷气就加重。多见于刚吹风着凉、感冒初起。方向是疏风散寒、宣肺止咳——这时候如果猛喝凉茶,反而雪上加霜。
A heavy cough sound, clear-thin and fairly copious phlegm, blocked nose with runny clear mucus, aversion to cold, worse after chill or aircon. Common at the onset of a cold. The direction is to disperse wind-cold and restore lung function — reaching for cooling herbal tea here only makes it worse.
Bunyi batuk berat, kahak cair dan banyak, hidung tersumbat, takut sejuk, lebih teruk selepas kesejukan atau penyaman udara. Biasa pada permulaan selesema. Arahnya menyuraikan angin-sejuk — minum teh herba sejuk di sini memburukkannya.
干咳少痰,或痰黄黏不易咳出,喉咙痒痛、口干,鼻涕偏黄。冷气房、烟霾季、天气干燥时最常见。方向是疏风清热、润燥止咳——干咳的重点是「润」,不是拼命化痰。这类在马六甲冷气环境里特别多。
A dry cough with scant phlegm, or yellow sticky phlegm that's hard to bring up, itchy or sore throat, dry mouth, yellowish mucus. Most common in aircon rooms, haze season, and dry spells. The direction is to clear heat and moisten — for a dry cough the key is moistening, not forcing phlegm out. Very common in Melaka's air-conditioned environments.
Batuk kering dengan sedikit kahak, atau kahak kuning melekit, tekak gatal, mulut kering. Paling biasa dalam bilik berhawa dingin dan musim jerebu. Arahnya menyejukkan haba dan melembapkan — bagi batuk kering, kuncinya melembapkan.
痰多、色白、容易咳出,胸口闷、身体沉重,喉咙常觉得有痰黏着,食欲差。根源常在脾——脾虚生湿、湿聚成痰,「脾为生痰之源,肺为贮痰之器」。爱吃生冷、油腻、甜食的人尤其明显。这类光润肺没用,要健脾化痰。
Copious white phlegm that comes up easily, a tight chest, a heavy body, a constant sense of phlegm stuck in the throat, poor appetite. The root is often the spleen — spleen deficiency breeds damp, damp gathers into phlegm ("the spleen is the source of phlegm, the lung merely stores it"). Pronounced in those who love cold, greasy, or sweet food. Moistening the lung alone won't help — the spleen needs strengthening.
Kahak banyak berwarna putih yang mudah keluar, dada sesak, badan berat, rasa kahak melekat di tekak, selera makan lemah. Puncanya sering limpa. Ketara pada mereka yang suka makanan sejuk, berminyak, atau manis. Melembapkan paru-paru sahaja tidak membantu.
咳了几周甚至几个月,咳声无力、气短,稍微动一下、讲话多一点就咳,容易累,晚上或清晨咳得明显。多是外感失治拖久,或本身体质偏虚。方向是补益肺气、滋养肺阴,一边扶正一边清余邪,需要较长的调理周期。
Coughing for weeks or even months, a weak cough sound, shortness of breath, coughing after slight exertion or too much talking, easy fatigue, worse at night or dawn. Usually an acute cough left untreated too long, or an already-weak constitution. The direction is to tonify lung qi and nourish lung yin while clearing any residual pathogen — this takes a longer course.
Batuk selama berminggu atau berbulan, bunyi batuk lemah, sesak nafas, batuk selepas sedikit aktiviti, mudah penat, lebih teruk pada waktu malam. Biasanya batuk akut yang dibiarkan terlalu lama. Arahnya menguatkan qi paru-paru — ini mengambil masa lebih lama.

中医处理咳嗽的核心,不是硬压止咳,而是先分清寒、热、燥、湿、虚,再决定该驱、该清、该润、该化、还是该补。方向对了,咳嗽是「顺势退场」,而不是被强行压下去。
The core of TCM's approach isn't to force the cough down, but to first tell apart cold, heat, dryness, damp, and deficiency, then decide whether to disperse, clear, moisten, resolve phlegm, or tonify. Get the direction right and the cough eases out naturally, rather than being suppressed.
Teras pendekatan TCM bukan menekan batuk secara paksa, tetapi mengasingkan sejuk, panas, kering, lembap, dan kekurangan dahulu, kemudian menentukan arah. Arah yang betul menjadikan batuk reda secara semula jadi.
风寒用三拗汤、杏苏散疏风散寒;风热用桑菊饮疏风清热;燥咳用桑杏汤、清燥救肺润燥;痰湿用二陈汤、三子养亲汤健脾化痰;久咳肺虚用百合固金、补肺类滋阴补气。核心永远是先辨证——同样是止咳,寒热用反了效果天差地别。
Sanao Tang or Xingsu San for wind-cold; Sang Ju Yin for wind-heat; Sang Xing Tang or Qingzao Jiufei for dry cough; Erchen Tang or Sanzi Yangqin for phlegm-damp; Baihe Gujin or lung-tonifying formulas for chronic lung deficiency. The core is always pattern differentiation first — the same "cough relief" applied to the wrong cold/heat pattern gives wildly different results.

Acupuncture commonly uses Feishu, Lieque, Chize, and Tiantu to open the lung's qi and ease the cough and throat itch. Wind-cold and phlegm-damp patterns add gliding cupping or cupping along the back Bladder meridian to disperse cold and resolve phlegm — many feel the chest loosen right after. Paired with acupuncture, acute cases clear faster.
Akupunktur biasanya menggunakan Feishu, Lieque, Chize, dan Tiantu untuk melegakan batuk. Corak angin-sejuk dan lembap-kahak menambah bekam di belakang untuk menyuraikan sejuk dan kahak.
急性外感的咳,方向对了往往几剂药、几天内就见效。久咳缠绵的,一般需要2–4周,让咳的程度减轻、次数减少、夜咳和晨咳改善,慢慢把肺气养回来。目标是让身体自己把余邪清干净、肺气恢复,而不是靠止咳药一直压着。
For an acute cough, the right direction often shows results within a few doses and a few days. A lingering chronic cough usually needs 2–4 weeks to reduce the severity and frequency, ease the night and morning coughing, and gradually rebuild lung qi. The goal is for the body to clear the residual pathogen and restore lung function itself — not to keep it suppressed with cough medicine.
Untuk batuk akut, arah yang betul sering menunjukkan hasil dalam beberapa hari. Batuk kronik biasanya memerlukan 2–4 minggu. Matlamatnya adalah untuk badan memulihkan fungsi paru-paru sendiri.
一个安全底线,请一定记住:咳嗽不该无限拖。「感冒好了还咳两三周」很多人当小事,但久咳最容易由实转虚、耗伤肺气,越拖越难调。更重要的是——如果咳嗽超过3周仍不见好、痰中带血、伴随明显消瘦、持续低烧、夜间盗汗或呼吸困难,一定要及时就医做进一步检查(不只是看中医),排除肺部其他问题。这不是吓你,是该有的谨慎。中医调理与必要的西医检查,从来不冲突。
One safety line to remember: a cough shouldn't be left indefinitely. "Still coughing weeks after a cold" is treated as trivial by many, but a lingering cough easily shifts from excess to deficiency and drains lung qi. More importantly — if a cough lasts beyond 3 weeks without improving, has blood in the phlegm, comes with marked weight loss, persistent low fever, night sweats, or breathing difficulty, seek medical assessment promptly (not only TCM) to rule out other lung conditions. This isn't scaremongering — it's sensible caution. TCM care and necessary medical investigation never conflict.
Satu garis keselamatan: batuk tidak patut dibiarkan tanpa had. Jika batuk berlarutan melebihi 3 minggu tanpa penambahbaikan, ada darah dalam kahak, disertai penurunan berat badan ketara, demam rendah berterusan, peluh malam, atau kesukaran bernafas, dapatkan penilaian perubatan dengan segera (bukan hanya TCM). Penjagaan TCM dan pemeriksaan perubatan tidak pernah bercanggah.

- 观察并记下你咳嗽的规律。有痰还是干咳?痰是白稀的还是黄稠的?什么时候咳得最厉害(晨起、夜间、受凉、说话多)?什么会诱发或加重?看诊时说清楚这几点,医师判断证型会又快又准,比只说「我一直咳」有用得多。 Observe and note the pattern of your cough. With phlegm or dry? - 护住脖子和胸口,避开诱因。冷气房里备一件外套、围一条薄围巾,别对着冷气风口直吹;烟霾天减少户外活动、外出戴口罩。这些看似小事,却能明显减少咽喉受刺激、减少咳嗽发作,也让调理不被反复诱发拖后腿。 Protect the neck and chest, and avoid triggers. Keep a jacket or light scarf in air-conditioned rooms, and don't sit directly under the vent; on hazy days, cut outdoor activity and wear a mask outside. Small things, but they noticeably reduce throat irritation and coughing fits — and stop constant triggers from undermining treatment. Lindungi leher dan dada, elak pencetus. - 别一咳就狂喝凉茶,喝对才有帮助。凉茶偏寒,只适合风热、燥热、热痰型;如果你是风寒咳(痰白、怕冷)或痰湿咳(脾虚湿重),猛灌凉茶反而助寒助湿,越喝越糟。干咳喉痒可温水小口多喝、室内放盆水加湿;痰多的则别再碰冰的、甜的、油腻的。忌口按证型来,不是人人都该猛喝凉茶。 Don't down herbal cooling tea at the first cough — only the right kind helps. Cooling tea is cold in nature and suits only wind-heat, dryness, and hot-phlegm types. If you have a wind-cold cough (clear phlegm, feeling cold) or phlegm-damp cough (weak spleen, heavy damp), gulping cooling tea adds cold and damp and makes it worse. For a dry, itchy cough, sip warm water often and humidify the room; if you have lots of phlegm, avoid cold, sweet, and greasy food. Match the diet to your pattern — cooling tea isn't for everyone. Jangan minum teh herba sejuk pada batuk pertama — hanya jenis yang betul membantu. Teh sejuk sesuai hanya untuk jenis angin-panas dan kahak-panas.
