You wake up, or within a day or two, one side of your face suddenly "won't obey" — the mouth pulls toward the good side, the eye on the affected side won't close, the forehead won't lift, your smile is lopsided, water leaks from the corner of your mouth when you rinse, food packs between teeth and cheek, and some people also get pain behind the ear or a changed sense of taste. This is most likely facial palsy (peripheral facial-nerve paralysis; the commonest form is Bell's palsy), often triggered by a cold draught or a virus (such as herpes) causing the facial nerve to swell. The good news: facial palsy is one of acupuncture's most classic and effective indications, and the earlier you start (especially within one to two weeks of onset), the better the recovery. With a "drooping mouth," the crucial first step isn't to rush into needles — it's to tell whether this is facial palsy (peripheral) or a stroke (central), because the response is entirely different and a stroke is a race against the clock. The key clue: if the drooping comes with weakness in an arm or leg, slurred or unintelligible speech, spinning dizziness, a severe headache, or confusion, treat it as a possible stroke — call an ambulance and go to hospital at once, do not come to a TCM centre first. Another simple check: in peripheral facial palsy the affected side can't lift the forehead or wrinkle the brow, whereas in a stroke-related droop the forehead usually still wrinkles. Once facial palsy is confirmed, get a medical diagnosis first — to rule out middle-ear infection, shingles, a tumour and the like; in the acute phase doctors often prescribe steroids (sometimes an antiviral), which you should take — TCM and Western care can run in parallel and don't conflict. Acupuncture works to a "golden window": the acute phase (about the first week) through the recovery phase (roughly two weeks to three months) is the key window for acupuncture to aid recovery, and most people improve markedly within it; drag past three to six months into "stubborn facial palsy" and recovery is much slower. If it hasn't recovered past three to six months, it's stubborn facial palsy — slower to improve, and watch for "synkinesis" (e.g. the mouth twitching when you blink). It can still be worked on, but with patience, aiming to improve as much as possible and avoiding over-strong stimulation. So don't delay — sooner is better.

Peripheral facial palsy affects one whole side of the face, including the forehead (which can't lift) and an eye that won't close, and it usually happens on its own. A stroke (central) usually still lets the forehead wrinkle, and often comes with limb weakness, slurred speech, a severe headache or confusion. Rule of thumb: if the drooping comes with weakness in an arm or leg, slurred speech, spinning dizziness or confusion, treat it as a possible stroke — call an ambulance and go to hospital, don't come to a TCM centre first. Protect it actively: use artificial tears by day, wear an eye patch or cover the eye at night (or as your doctor advises), and wear sunglasses outdoors against wind and dust. Keep the face warm and stop letting cold wind or air-con hit the affected side. In the acute phase doctors often prescribe steroids (sometimes an antiviral) — take them as directed; TCM acupuncture can run alongside, they don't conflict.
