反复崴脚,多半不是你「不小心」,而是第一次扭伤后韧带没养好、脚踝的稳定和本体感觉退化了——路面稍微不平、下个楼梯没踩稳,就又翻过去。所以中医处理反复崴脚,重点不只在当下消肿止痛,更在把踝关节的「稳」练回来:先按分期把这一次处理好,再养回韧带、肌力和平衡感,才能打断「崴了又崴」的循环。这篇讲清楚:崴脚到底扭到了什么、急性期该做和不该做什么、为什么会变成习惯性崴脚,以及什么情况必须先照X光看西医。

A recurring ankle sprain usually isn't down to being "careless." It's that after the first sprain the ligament never fully healed, and the ankle's stability and proprioception faded — so a slightly uneven pavement or a missed step rolls it again. That's why TCM care aims beyond just settling the swelling and pain: it works to rebuild the ankle's "steadiness" — treat this episode by stage, then restore ligament, strength and balance to break the sprain-again cycle. This piece covers what actually gets injured, what to do (and not do) in the acute stage, why it turns into a chronically unstable ankle, and when you must get an X-ray first.

Terseliuh berulang selalunya bukan sebab anda "cuai." Selepas terseliuh pertama, ligamen tak sembuh sepenuhnya, dan kestabilan serta proprioseptif buku lali merosot — jadi jalan tak rata sedikit pun ia terpusing semula. 最常见的崴脚,是脚掌向内翻(内翻扭伤)——脚一歪,外脚踝那一侧的韧带被过度拉扯,受伤的多是外踝前方的「距腓前韧带」。这也是为什么大部分人崴的是外侧、外脚踝下方肿一大包。少数是脚往外翻伤到内侧,或扭到小腿两根骨头之间的高位韧带,这类往往更久才好,需要医师分辨。

The most common sprain is the foot rolling inward (an inversion sprain) — as it tips, the ligaments on the outer ankle are overstretched, most often the anterior talofibular ligament in front of the outer ankle bone. That's why most people sprain the outside and swell up just below the outer ankle. Less often the foot rolls outward and hurts the inner side, or a "high ankle" ligament between the two shin bones is strained — these tend to take longer and need a practitioner to tell apart.

Sprains are graded by ligament damage: Grade I — the ligament is overstretched but not torn, mild swelling and pain, usually still walkable; Grade II — partial tear, clear swelling and bruising, painful and a little unstable; Grade III — a full rupture, a very unstable ankle that swells fast and hard, often needing an X-ray to rule out an accompanying fracture or avulsion. Recovery differs greatly — a Grade I may take a week or two, a Grade III several months with more careful support. The worse it is, the sooner it should be assessed — don't judge this one by "last time was like this too."

This is the most important part of the article. Many people resume activity once the pain fades, but the ankle hasn't truly "healed." Recurring sprains usually come from three things stacking up — understanding them shows why easing pain and swelling alone isn't enough.

If the first sprain isn't rested and you return to hard activity too soon, the stretched ligament heals looser and weaker — the ankle's "brake" isn't tight enough. A loosened ligament won't tighten back on its own; stability training has to compensate.

The ankle has a "position sense" that constantly tells the brain what angle the foot is at. A sprain disrupts it, slowing the reaction — step on uneven ground and the foot rolls before the brain can tighten things. This is the most overlooked link in recurring sprains — and the most trainable.

A forceful roll can nudge the talus, calcaneus, or the cuboid in the midfoot slightly out of place and jam it, leaving movement "not quite right." This may not settle on rest alone; a bone-setting adjustment to guide it back can make the ankle move and hold more smoothly.

Pusingan kuat boleh menganjak talus, kalkaneus atau kuboid sedikit dan menyekatnya, meninggalkan pergerakan "tak berapa betul." Ini mungkin tak reda dengan rehat sahaja; pelarasan tulang boleh membantunya kembali betul.

Grasp these four and it's clear: a recurring sprain is about the root of the instability, not just this episode's swelling and pain. Tending the ligament, proprioception, strength and joint position together is what stops the cycle.

崴脚当下的处理,很大程度决定它好得快不快。这里有一个本地很常见的误区要先纠正——刚崴脚,别急着热敷、别马上大力推拿、别猛涂药油「揉散」它。

How you handle a fresh sprain largely decides how fast it recovers. One very common local habit needs correcting first — on a fresh sprain, don't rush to apply heat, don't have it forcefully massaged, and don't vigorously rub in medicated oil to "break it up."

Cara anda menangani terseliuh baru banyak menentukan kelajuan pemulihan. Satu tabiat tempatan perlu dibetulkan dahulu — pada terseliuh baru, jangan tergesa-gesa bakar/panaskan, jangan diurut kuat, dan jangan sapu minyak dengan kuat untuk "menyahkannya."

On a fresh sprain, the ligament and small vessels are bleeding and the tissue is swelling. Heat and hard rubbing widen the vessels, bleed more, and swell worse — slowing recovery. The first 48–72 hours follow the universal PRICE: Protect (no re-injury), Rest (relative), Ice (about 15–20 min, through a towel), Compression (elastic bandage), Elevation (above the heart). Once these days pass and the swelling settles into the repair stage, a practitioner judges when warming, blood-moving and hands-on release are appropriate. "Rub it warm to move the bruise" belongs to the repair stage, not the acute one — wrong stage, wrong help.

中医看崴脚,急性期是「气滞血瘀、肿痛不通」,重在消肿止痛、别扰动;到了修复期才「温通活血、松解」;最后是「养肝肾、壮筋骨、把稳练回来」。方向对了,好转是肿消得快、走路稳一点、不再一踩就发软、复发次数变少

In TCM terms, a fresh sprain is "qi and blood stagnation — swollen, painful, blocked," so the priority is to reduce swelling and pain without disturbing it; the repair stage then "warms, moves blood, and releases"; finally comes "nourishing the liver and kidney, strengthening the sinews and bone, and retraining steadiness." Get it right and improvement shows as faster de-swelling, steadier walking, less giving-way, and fewer repeat sprains.

Dari segi TCM, terseliuh baru ialah "qi dan darah tersekat — bengkak, sakit, tersumbat," jadi keutamaannya mengurangkan bengkak dan sakit tanpa mengganggunya; peringkat pemulihan kemudian "memanaskan, menggerakkan darah, dan melepaskan"; akhirnya "memupuk hati-buah pinggang, menguatkan urat dan tulang." 急性期常用「远端取穴」——在离伤处较远的位置下针(如对侧或膝部、健侧相应点),以及踝周未肿的穴位,帮助止痛、促进循环消肿,而不去刺激正在出血肿胀的地方。配合冰敷、加压、抬高,让肿痛先降下来。这个阶段绝不在肿处大力推拿或热敷。

The acute stage uses distal points — needling away from the injury (the opposite side, the knee, matching points) and un-swollen points around the ankle — to ease pain and help circulation drain the swelling, without provoking the actively bleeding, swollen area. Paired with ice, compression and elevation to bring the swelling down first. No hard massage or heat on the swelling at this stage.

肿势稳住后,转为帮它「化开、松开、归位」。针灸配艾灸温通、活血化瘀,帮助瘀血吸收;轻柔的推拿、理筋松开扭伤后紧绷、粘连的软组织;若评估有距骨、骰骨等小关节错位,用正骨手法把它调回正确位置——脚踝往往当场就顺一点、活动度大一点。虚寒瘀滞明显的,配合科学中药(浓缩颗粒)从内在帮气血运行、筋骨修复。

Once the swelling settles, care shifts to "dispersing, loosening, realigning." Acupuncture with moxibustion warms and moves blood to help the bruise clear; gentle tuina and sinew work free the tight, adhered soft tissue after the sprain; if a talus or cuboid is assessed as jammed, a bone-setting adjustment guides it back — the ankle often moves a little more smoothly on the spot. For clear cold-stagnation patterns, concentrated herbal granules support circulation and tissue repair from within.

这一步决定它会不会再崴。中医认为肝主筋、肾主骨,会以补肝肾、强筋骨的方向调理体质,让筋骨更「耐用」。更关键的是配合本体感觉与肌力训练——从双脚站稳,到单脚站、闭眼单脚站、软垫上站,一步步把脚踝的平衡反应和腓骨肌力量练回来。这是把「崴了又崴」真正打断的一步,也是最需要你在家坚持的部分。

This step decides whether it sprains again. TCM holds that the liver governs the sinews and the kidney the bones, so care leans toward tonifying the liver and kidney to make the sinews and bones more durable. More crucially, it pairs with proprioception and strength training — from standing on two feet, to single-leg stands, eyes-closed stands, and standing on a cushion — step by step rebuilding the ankle's balance reflex and peroneal strength. This is what truly breaks the sprain-again cycle, and the part that most needs your persistence at home.

One safety line — see a doctor and get imaging first for these. Most ankle sprains can be worked through calmly by stage, but with any of the following, head to A&E or orthopaedics first to rule out a fracture or full ligament rupture: a clear "pop" at the moment of injury; being unable to bear weight for four steps both then and now; bony tenderness on the tip of the outer or inner ankle bone or specific midfoot bones (the clinical "Ottawa ankle rules" for whether an X-ray is needed); an ankle that swells huge fast or looks deformed; toes or foot going numb, pale, or cold (possible vessel or nerve pressure). Also, a single hot, red, swollen joint with no real sprain history may be gout or a joint infection — an entirely different direction. TCM care never conflicts with necessary imaging and support — image first when indicated, then treat with a clear picture.

  • 刚崴的头两三天,先冰敷、加压、抬高,别热敷别猛揉。用毛巾裹着冰袋敷15–20分钟、用弹性绷带适度加压、把脚垫高过心脏,能有效控制肿胀。这几天别泡热水、别涂药油大力揉、别让人用力推拿患处——把「揉热散瘀」留到肿退之后由医师判断。真的痛得厉害、肿得快,先去照影像。 For the first two or three days, ice, compress and elevate — no heat, no hard rubbing. Ice through a towel for 15–20 minutes, apply moderate compression with an elastic bandage, and raise the foot above the heart to control swelling. Avoid hot soaks, vigorous oil-rubbing, and forceful massage of the injury these days — leave "rub it warm" until the swelling drops and a practitioner judges. If pain is severe or swelling is fast, get imaging first. Dua tiga hari pertama, ais, mampat dan tinggikan — jangan panas, jangan urut kuat. Ais berlapik tuala 15–20 minit, mampat sederhana dengan pembalut elastik, dan tinggikan kaki atas jantung. Elak rendaman panas dan urutan kuat hari-hari ini.   • 记下「怎么崴的」和旧伤史。看诊前想清楚:怎么崴的(往内翻还是往外翻、有没有听到声音)、当下能不能走、肿在外踝还是内踝、以前这只脚崴过几次、上次多久才好。反复崴脚的人,这段病史特别重要——它帮医师判断是不是已经出现慢性不稳,要不要把稳定训练排进疗程。 Note how it happened and your history. Before the visit, get clear on: how it rolled (inward or outward, any sound), whether you could walk, whether the swelling is outer or inner, how many times this ankle has sprained before, and how long last time took. For repeat sprainers this history matters most — it helps the practitioner judge whether chronic instability has set in and whether stability training belongs in the plan. A sole worn down on one edge, too-soft unsupportive shoes, an oversized fit, or flip-flops and heels on uneven ground all make an already-unstable ankle roll more easily. During recovery, switch to a supportive, well-fitted shoe; take stairs, slopes and dim spots slowly and watch your footing.
