「医师,我这个大概几次会好?」几乎每个病人都问过。很多人心里的期待是「最好一次搞定」,如果一两次没立刻好,就开始怀疑是不是没效。其实,中医通常需要「一个疗程」,不是拖延,而是因为身体的问题多半是长期累积的,调理也需要过程。这篇讲清楚疗程背后的道理,以及怎么判断调理到底有没有在起作用。
"How many sessions until I'm better?" Almost every patient asks. Many quietly hope to fix it in one go, and if a session or two doesn't bring instant relief, they start doubting it works. In truth, TCM usually needs a "course of treatment" — not to stall, but because the body's issues are mostly built up over time, and treatment needs a process. Here's the reasoning behind a course, and how to tell whether treatment is actually working.
"Berapa sesi sampai saya sembuh?" Hampir setiap pesakit bertanya. Ramai berharap membaikinya sekali gus. Sebenarnya, TCM biasanya perlukan "tempoh rawatan" — bukan untuk melengahkan, tetapi kerana masalah badan kebanyakannya terbina dari masa ke masa.

想想看:一个反复多年的腰痛、一个拖了很久的失眠或胃胀,往往是长年的姿势、劳损、作息、饮食一点点累积出来的。它不是一天形成的,自然也很难一次就解决。中医调理,是给身体足够的时间,把失衡一步步拉回来。
Consider: a back pain that's recurred for years, or long-standing insomnia or bloating, is usually built up little by little from years of posture, strain, routine and diet. It didn't form in a day, so it rarely resolves in one. A course of TCM gives the body enough time to bring the imbalance back, step by step.
Fikirkan: sakit belakang bertahun, atau insomnia lama, biasanya terbina sedikit demi sedikit dari postur, regangan dan diet bertahun. Ia tidak terbentuk dalam sehari.
还有一个更根本的原因:中医治的是「证」,而证会随身体状态变化。这次是实证,调理一段后可能转虚,方就要跟着改。所以中医强调复诊——每次都重新辨证、校准方向,一张方子从头吃到尾反而不符合中医的逻辑。疗程,正是这个动态调整的过程。(详见 中医看的是「证」不是「病」)
There's a deeper reason too: TCM treats the "pattern", and the pattern shifts with the body's state. An excess pattern now may become deficient after some treatment, and the prescription must change with it. This is why TCM emphasises follow-ups — re-assessing and recalibrating each time; taking one unchanged formula start to finish actually goes against TCM's logic. 像腰痛、颈椎、膝盖这类,通常先在急性期较密集地处理,把疼痛和炎症降下来;缓解后再拉长间隔,进入维持巩固阶段。要诚实说:劳损类的症状会倒回来,只是时间问题——目标是让它程度更轻、次数更少、时间更短。
For back, neck or knee pain, treatment is usually more intensive during the acute phase to bring down pain and inflammation, then spaced out into a maintenance phase. Honestly: strain-type symptoms will come back — it's a matter of time. The goal is to make them milder, less frequent and shorter.
Untuk sakit belakang, leher, lutut, rawatan lebih kerap pada fasa akut, kemudian dijarakkan. Jujurnya: gejala regangan akan kembali — matlamatnya buat ia lebih ringan, jarang dan singkat.
像失眠、胃胀、月经、体质这类,通常先用大约一周的中药,看身体的反应、确认方向对不对。方向对了就持续,需要微调就调方。体质层面的改变通常需要3–6个月,急不来,但方向对了会一步步靠近。
For insomnia, bloating, menstruation or constitution, treatment usually starts with about a week of herbs to see the body's response and confirm the direction. If it's right, continue; if it needs tweaking, adjust. Constitutional change usually takes 3–6 months — it can't be rushed, but the right direction moves you closer step by step.
Untuk insomnia, kembung, haid atau perlembagaan, rawatan bermula dengan ~1 minggu herba untuk sahkan arah. Perubahan perlembagaan biasanya 3–6 bulan.
别用「一次断根」当标准,看方向性好转

很多人判断有没有效的标准是「有没有一次好透」,这其实不太符合身体恢复的规律。更实际、也更准确的标准,是看方向性好转——就是上面那三个维度:
Many judge effectiveness by "did it fully clear in one go", which doesn't match how the body actually recovers. A more practical and accurate standard is directional improvement — the three dimensions above:
Ramai menilai keberkesanan dengan "sembuh sepenuhnya sekali gus", yang tidak sepadan dengan cara badan pulih. Piawai lebih praktikal ialah peningkatan berarah — tiga dimensi di atas:
- 程度更轻:发作起来没以前那么重、那么难受。Less severe: flare-ups aren't as heavy or as hard to bear as before.Kurang teruk: serangan tidak seberat dahulu.
- 次数更少:发作之间的间隔变长,出现得没那么频繁。Less frequent: the gaps between episodes lengthen; it happens less often.Kurang kerap: jarak antara episod memanjang.
- 时间更短:每次不舒服持续的时间缩短,好得比以前快。Shorter: each episode lasts a shorter time and eases faster than before.Lebih singkat: setiap episod lebih pendek dan pulih lebih cepat.
也别「好一点就停」。症状缓解了,不代表背后的证已经稳了——太早停,容易反复,前面的调理就打了折扣。就像劳损修复到一半停手,很快又倒回去。是否可以结束或调整疗程,最好交给医师,根据你的脉象、舌象和整体状态判断,而不是凭「感觉好多了」就自己决定。当然,我们也不会用「几次断根、保证根治」这种说法——身体的恢复有它的节奏,我们做的是把方向和预期,如实告诉你。
And don't stop the moment you feel better. Relief doesn't mean the underlying pattern has stabilised — stopping too early invites recurrence, discounting the progress made, like stopping halfway through repairing a strain only for it to slip back. Whether to end or adjust a course is best left to the practitioner, based on your pulse, tongue and overall state — not decided on "feeling much better" alone. And we won't use phrases like "cured in a few sessions, guaranteed" — the body recovers at its own pace, and what we do is tell you the direction and expectations honestly.
Dan jangan berhenti sebaik rasa lebih baik. Kelegaan tidak bermakna corak telah stabil — berhenti terlalu awal menjemput berulang. Sama ada mengakhiri tempoh sebaiknya diserahkan kepada pengamal. Kami tidak akan guna frasa "sembuh dalam beberapa sesi, dijamin".

At An Xin TCM, a course isn't a fixed number set in stone at the start, but adjusts dynamically with the body's response. At each follow-up, the practitioner re-takes the pulse, checks the tongue and re-differentiates the pattern to see if the direction holds, then decides the next step. We try to set expectations honestly — roughly how long, which stages to expect, what counts as improving — rather than giving an inflated promise.
