Gua sha is probably the most misunderstood — and most easily overdone — of all the TCM external therapies. This piece has one central point, and we'll put it bluntly: gua sha is not a case of the more it hurts, the better it works. Excessive force doesn't double the benefit; it just damages the tissue under your skin. We'll cover what gua sha actually does, how firm is firm enough, how it divides labour with cupping, what it suits, who it doesn't, and why you really shouldn't scrape yourself at home with a coin or a spoon.

In gua sha, the practitioner applies a lubricant — an oil or balm — then uses a smooth, blunt-edged tool (buffalo horn, bian stone, jade) to stroke repeatedly in one direction over a chosen area, raising red to purplish petechiae under the skin: what's called "sha." Its direction of action is to move qi and blood, release an exterior pattern, and ease fascial tension — dispersing what's stuck and congested near the surface.

先说正确答案:刮痧的力度应该是「有感觉,但可以忍受」。多数人形容起来是被钝钝地推过去、有点酸胀、有点热——不是龇牙咧嘴、抓着椅子、痛到要叫出来。如果你需要靠意志力撑过去,那不是「有效」,那是过头了。
The correct answer first: gua sha pressure should be "you feel it, but you can tolerate it." Most people describe a blunt push, a dull ache, some warmth — not gritted teeth, gripping the chair, yelping. If you need willpower to get through it, that isn't "effective" — that's overdone.

它来自一个看起来很合理、其实倒因为果的推论:「用力 → 出痧多 → 所以出痧多是用力刮出来的 → 所以用力=有效」。问题出在中间那一步。出痧多少,主要取决于你个人的体质和那个部位当下的气血状态——同样的手法、同样的力度,有人一刮就一片红紫,有人怎么刮都只是淡淡的粉红。这跟医师用了多大力气关系不大,跟你「有多需要」也不是简单的正比。医师真正在读的,是痧出现的分布、快慢、部位,那是诊断讯息,不是打分表。所以用「今天刮得比较痛,应该比较有用」来自我安慰,是把因果接反了。
It comes from a reasonable-looking inference that actually reverses cause and effect: "press hard → lots of sha → therefore lots of sha comes from pressing hard → therefore hard equals effective." The error is in the middle step. How much sha appears depends mainly on your constitution and the state of qi and blood in that area right now — same technique, same pressure, and one person comes up red-purple immediately while another stays faintly pink no matter what. It has little to do with how hard the practitioner pushed, and it isn't a simple proportion to "how much you needed it" either. What the practitioner is actually reading is where the sha appears, how fast, and in what distribution — that's diagnostic information, not a score sheet. So reassuring yourself with "it hurt more today, so it must have worked better" has the causation backwards.

So what does excessive force actually do? It doesn't give you more benefit — it gives you more injury. The force behind that tool is ultimately absorbed by the small vessels and soft tissue under your skin. When pressure goes unchecked, the usual results are excessive bruising, pain lasting several days, local swelling, and broken skin — and broken skin carries infection risk. You went in to loosen a shoulder and came out unable to be touched for a week. Worse, many people read those few sore days as "a normal detox reaction," and so next time they go looking for someone who scrapes even harder, and the cycle deepens.

Remember these three lines. One: gua sha is about freeing qi and blood, not tearing up your skin. The aim is dispersal, not creating a wound. Two: "hurts enough to yelp" is not the standard — "you feel it but can take it" is. You can say "that's too hard" at any point, and a good practitioner adjusts immediately rather than telling you to endure it. Anyone displeased that you asked them to stop is someone you should stop seeing. Three: how much sha appears isn't up to you, and it isn't up to the practitioner's strength either. Some constitutions simply don't raise much sha, and scraping until it appears is spending your skin to satisfy a mistaken expectation. The value of gua sha lies in getting it right, not in going hard.

Many people assume these are "much the same thing — just pick one." In fact they act differently, and in clinic they're often used together. The simplest split: gua sha covers a surface; cupping holds a point.

Stroking along the channels across a whole region: wide coverage, oriented to dispersing the exterior. It suits the state where "I can't point to one spot — the whole shoulder and back is stiff and congested," or an early exterior pattern, or summer-damp weighing the whole person down. Its strength is breadth — one pass covers a lot of ground.
Menyapu mengikut saluran merentasi satu kawasan: liputan luas, berorientasikan menyurai lapisan luar. Sesuai untuk keadaan "saya tak boleh tunjuk satu tempat — seluruh bahu dan belakang kaku dan sesak." Kekuatannya ialah keluasan.

Suction held on a fixed point for a stretch of time, acting relatively deeper and more concentrated. It suits a defined knot — "right there, that spot is exquisitely sore when you press it" — or deeper stasis. Its strength is focus: concentrated attention on one location.
Sedutan dipegang pada titik tetap untuk satu tempoh, bertindak lebih dalam dan lebih tertumpu. Sesuai untuk ketulan yang jelas — "di situ, tempat itu sangat lenguh bila ditekan" — atau takungan yang lebih dalam.
所以不是二选一,而是看你的状况需要「铺开」还是「钻进去」——很多时候医师会先刮痧把一整片松开,再针对剩下那几个顽固的点拔罐,或者配合推拿处理更深的层次。用哪个、怎么配、刮多大范围、留罐多久,都由医师按证型和部位决定,这也是「由医师在中心做」和「在家自己乱来」最实质的差别。
So it isn't either/or — it's whether your situation needs spreading out or drilling in. Often the practitioner will use gua sha to loosen a whole region first, then cup the few stubborn points that remain, or bring in tuina for the deeper layers. Which one, how they're combined, how wide an area, how long a cup stays — all decided by the practitioner according to pattern and location. That's the substantive difference between having it done at a centre and improvising at home.

临床上比较常用在这几类:外感初起(刚开始怕冷、头重、肩颈紧、身体酸,还没真正病下去的那个阶段);暑湿困阻(在马来西亚特别常见——闷热天里头昏沉、身体重、胸口闷、胃口差);大面积的肩背僵硬(长期伏案、冷气直吹,整片斜方肌又硬又闷);以及某些筋膜张力过高的情况。共同点是:范围偏大、偏表层、有「闷住散不开」的感觉。
Its most common clinical uses: early exterior patterns (the stage where you're just starting to feel chilly, head heavy, neck and shoulders tight, body aching — before you've properly come down with anything); summer-damp obstruction (very common in Malaysia — a foggy head, heavy body, tight chest, poor appetite in the muggy heat); broad shoulder and upper-back stiffness (long hours at a desk with the aircon on you, the whole trapezius hard and congested); and certain presentations of excessive fascial tension. What they share: a larger area, nearer the surface, with that "congested and won't disperse" quality.

The practitioner takes your history, checks tongue and pulse, and confirms whether gua sha fits, where, and over how wide an area — while ruling out contraindications: clotting issues, blood thinners, pregnancy, the state of your skin. This step governs everything that follows.

Most people feel a blunt push and a dull ache, with some warmth.

擦净残油,医师交代护理:当天不洗冷水澡、不吹冷气直吹刮过的部位、不立刻剧烈运动或喝酒,喝点温水。痧痕一般几天到一周自然消退,期间不要搓揉。两次刮痧之间要留够间隔,等痧退了再刮下一次,不要天天刮。
The residual oil is wiped off and the practitioner explains aftercare: no cold shower that day, don't sit with the aircon blowing on the treated area, no heavy exercise or alcohol right after — have some warm water. The sha marks generally fade on their own over a few days to a week; don't rub them in the meantime. Leave enough of a gap between sessions — wait for the sha to fade before the next one. Not every day.
Minyak dilap dan pengamal menerangkan jagaan: jangan mandi air sejuk hari itu, jangan biarkan penghawa dingin terus pada kawasan itu, elak senaman berat atau alkohol — minum air suam. 安全底线:这些人不适合刮痧。凝血功能障碍、血小板过低、正在服用抗凝血药(warfarin、长期 aspirin)——刮痧本身就是制造微出血,这类人风险明显偏高;皮肤有破损、溃疡、感染、湿疹发作中、严重晒伤处不可刮;孕妇的腰骶部与腹部必须避开,其余部位也应由医师评估;正在发烧且体虚、极度疲劳、空腹或酒后不宜;静脉曲张明显处、骨突处、大血管与淋巴结密集区须避开或用极轻手法;糖尿病末梢感觉减退、长者皮肤菲薄者须特别谨慎。最后这句请一定记住:别在家拿硬币、汤匙、瓶盖用力乱刮。这些东西边缘不够圆钝、没有润滑、力度没人控制——刮破皮、刮到瘀伤一片、刮在不该刮的地方,是家庭自刮最常见的三种意外。刮痧应由 KKM 注册中医师执行。安全,永远排在效果前面。
Safety line: these people shouldn't have gua sha. Clotting disorders, low platelets, or blood thinners (warfarin, long-term aspirin) — gua sha creates micro-bleeding by design, so the risk here is clearly elevated; broken, ulcerated, or infected skin, active eczema, or severe sunburn must not be scraped; pregnancy means the lumbosacral area and abdomen are off-limits, and other areas should be assessed by a practitioner; not when feverish and weak, exhausted, on an empty stomach, or after alcohol; prominent varicose veins, bony prominences, and areas dense with major vessels or lymph nodes should be avoided or handled with the lightest touch; and diabetes with reduced peripheral sensation, or thin elderly skin, needs particular care. And please remember this last line: don't scrape yourself at home with a coin, a spoon, or a bottle cap. Those edges aren't smooth enough, there's no lubricant, and nobody is controlling the pressure — broken skin, a sheet of bruising, and scraping somewhere you shouldn't are the three most common home-gua-sha accidents. Gua sha should be performed by a KKM-registered practitioner. Safety always comes before results.
