「我去看中医。」——这句话你说过很多次。但帮你的那个人,你有没有真的想过,他到底是中医师,还是推拿师、按摩技师、整骨师?你可能从来没问过,因为大家都这样去的,也没人告诉你要分。可是——当他在你身上扎针、扳你的脖子、给你开一包药的时候,他受过多少训练、能不能负这个责任,其实差很多。搞清楚你面对的是谁,能帮你避开不少误会,也让你更安心。
"I'm going to see a TCM practitioner." You've said it many times. But the person helping you — have you ever really wondered whether they're a TCM practitioner, a tuina therapist, a masseur, or a bone-setter? You may never have asked, because that's just how everyone goes, and no one tells you to. Yet when they needle you, move your neck, or hand you a packet of herbs, how much training they've had and whether they can carry that responsibility differ a great deal. Knowing who you're dealing with saves confusion and puts you more at ease.
"Saya nak jumpa pengamal TCM." Anda sering menyebutnya. Tetapi orang yang membantu anda — pernahkah anda terfikir sama ada dia pengamal TCM, ahli terapi tui na, tukang urut, atau tukang tulang? Namun apabila dia mencucuk jarum, menggerakkan leher, atau memberi anda herba, latihan dan tanggungjawab mereka amat berbeza.

这不是我们自己说了算的。自 2016 年起,马来西亚有《传统与辅助医药法令》(Act 775)。从 2016 年 8 月起,包括中医在内的传统与辅助医药执业,需要向「传统与辅助医药理事会」(T&CM Council)注册,前提是有被认可的学历。也就是说——把脉、辨证、开中药方,这些属于注册中医师的范畴,是有资格门槛的。而「帮你按一按、松一松」这类保健手法,又是另一回事。知道这条线在哪,你就知道自己该找谁。
This isn't just our say-so. Since 2016 the Traditional and Complementary Medicine Act 2016 (Act 775) has applied. From August 2016, T&CM practice including TCM requires registration with the T&CM Council, which rests on a recognised qualification. So pulse reading, pattern diagnosis and prescribing belong to a registered practitioner, with a real bar to clear. "Giving you a rub and a loosen" is another matter. Knowing where that line sits tells you who to look for.

受过系统训练(通常五年制),会把脉、看舌、辨证、开方,能判断你的问题属于哪一类、该不该转诊。针灸、开中药这类涉及判断和风险的事,属于他们的范畴。如果你的问题需要「弄清楚原因、对症调理」,你要找的是这一种。
Systematically trained (usually a five-year degree): reads pulse and tongue, differentiates, prescribes, and judges what your problem is and whether to refer. Acupuncture and herbs — things that carry judgement and risk — sit with them. If your problem needs the cause worked out and treated, this is who you want.
Terlatih secara sistematik (biasanya ijazah lima tahun): membaca nadi dan lidah, mendiagnosis, mempreskripsi. Jika masalah anda perlu punca difahami dan dirawat, inilah yang anda cari.
多做放松、保健类的手法,帮你松一松紧绷的肌肉、舒缓疲劳。训练重点在手法本身,通常不含辨证、诊断、开方。如果你只是累了、想放松一下,这很合适,也很有价值——只是它和「治疗一个说不清的问题」是两回事。
Mostly relaxation and wellness techniques — easing tight muscles and fatigue. The training centres on the technique itself and usually doesn't include differentiation, diagnosis or prescribing. If you're tired and want to unwind, this is a fine and valuable choice — it's just a different thing from treating a problem you can't quite name.
Kebanyakannya teknik relaksasi — melegakan otot tegang dan keletihan, biasanya tanpa diagnosis. Jika anda hanya penat dan mahu relaks, ini pilihan yang baik.
涉及关节、脊椎的调整,风险和所需训练都更高。中医的正骨、推拿,和西式的 chiropractic 又是不同体系。这类动作,尤其是颈部的猛力扳动,操作不当有真实风险——所以更要确认对方受过什么训练,有没有先判断你适不适合,而不是一上来就「咔」一声。
Working with joints and the spine — higher risk and higher training. TCM bone-setting and Western chiropractic are different systems again. These moves, especially a forceful neck manoeuvre, carry real risk if mishandled — so it matters even more to confirm the training, and whether they assessed if it suits you, rather than a straight-in "crack."

If what you want is to unwind and ease fatigue, tuina or massage is lovely. But if you have a problem that needs judgement and treatment — recurring pain, an internal complaint, a menstrual issue, a vague unwellness — then you want a registered practitioner who can diagnose, differentiate and take responsibility. At the clinic, you can gently ask: "Are you a registered practitioner?" Someone properly trained is usually glad to share.
