扭到脚、撞青一块、搬东西拉伤、打球摔了一跤——这些在马六甲常被统称为「跌打损伤」。很多人受伤后第一反应是赶快涂药油、用力揉、或去「跌打馆」热热地推开,但处理的时机和方法搞错了,反而会肿得更久、留下反复发作的旧患。中医看跌打损伤,核心是气滞血瘀、伤筋动络,关键在「分期处理」:急性期先消肿、别扰动,恢复期才散瘀通络,陈旧伤则温经通络。这篇讲清楚:跌打损伤到底指什么、头两三天该做和不该做什么、中医怎么分期调、以及什么情况一定要先照X光排除骨折。

A twisted ankle, a bruised knock, a strained back from lifting, a fall on the court — in Melaka these are loosely called "die-da" (traumatic injury). Many people's first instinct is to rub in medicated oil, massage it hard, or have it "worked warm" — but get the timing and method wrong and it swells for longer and leaves a recurring old injury. In TCM, traumatic injury is essentially qi-and-blood stagnation and injured sinews and channels, and the key is treating it by stage: in the acute stage settle the swelling without disturbing it; in the repair stage move and clear the stagnant blood; for old injuries, warm and free the channels. This piece covers what die-da really means, what to do (and not do) in the first days, how TCM treats it by stage, and when an X-ray to rule out a fracture must come first.

跌打损伤,泛指外力造成的筋肉、关节和软组织急性损伤——扭、撞、跌、挫、拉,都算。常见表现是局部肿胀、疼痛、瘀青、活动受限,严重的会动一下就痛。也包括当年受伤没处理好、留下来的陈旧伤——平时没事,一变天、一劳累、一用力就又酸又紧。中医看,外力让气血运行受阻、「离经之血」瘀在组织里,就是气滞血瘀、伤筋动络,瘀不散、络不通,就肿就痛就卡。

Die-da broadly means acute injury to the sinews, joints and soft tissue caused by external force — twists, knocks, falls, bruises, strains all count. It typically shows as local swelling, pain, bruising and limited movement, and when bad, pain on the slightest move. It also includes old injuries that were never properly treated — fine most days, but achy and tight again with a change in weather, fatigue, or exertion. In TCM, the force blocks the flow of qi and blood and leaves "blood outside its channels" stagnant in the tissue — that is qi-and-blood stagnation with injured sinews and channels: while the stagnation doesn't clear and the channels stay blocked, there is swelling, pain and stiffness.

Die-da secara luas bermaksud kecederaan akut pada urat, sendi dan tisu lembut akibat daya luar — pusingan, hentaman, jatuh, lebam, teregang semuanya dikira. Ia biasanya menunjukkan bengkak setempat, sakit, lebam dan pergerakan terhad. Ia juga termasuk kecederaan lama yang tak pernah dirawat dengan betul — baik biasanya, tetapi sakit dan ketat semula bila cuaca berubah, keletihan, atau mengangkat berat. 跌打损伤是一「类伤」,正骨是一「种手法」。跌打处理的是外力造成的筋肉软组织损伤(肿、痛、瘀、拉伤);正骨处理的是关节错位、小关节卡住、体态结构的问题。两者常有交集——比如重重一崴,既有软组织的跌打伤,可能也有踝骨小关节错位,需要正骨配合调回去。此外,「跌打」范围比「单一关节扭伤」更广:脚踝扭伤只是跌打损伤的其中一种,手腕、肩、腰、膝、肌肉拉伤、撞挫伤都在里面。到诊后,医师会辨证判断你这一次以哪一类为主、该怎么搭配针灸、手法、中药。

Die-da is a category of injury; bone-setting is a technique. Die-da deals with soft-tissue injury from force (swelling, pain, bruising, strain); bone-setting deals with a jammed or displaced joint and postural structure. They often overlap — a hard roll may carry both a soft-tissue injury and a slightly displaced ankle bone that bone-setting guides back. And "die-da" is broader than "a sprained ankle": the ankle is just one of many — wrists, shoulders, lower back, knees, muscle strains and bruises all belong here. At the visit, the practitioner works out which predominates this time and how to combine acupuncture, hands-on work and herbs.

Die-da ialah kategori kecederaan; urut tulang ialah teknik. Die-da menangani kecederaan tisu lembut akibat daya (bengkak, sakit, lebam, teregang); urut tulang menangani sendi yang tersekat atau teranjak dan struktur postur. Keduanya sering bertindih. A knock or a fall ruptures small vessels under the skin — bruising and a swollen lump. Most settle on their own, but a large, deep or still-growing one needs attention for deeper bleeding and tissue damage.

An old injury never fully cleared — stagnant blood left behind and channels never freed — leaves recurring aches and stiffness that flare with weather, damp or fatigue. Common in Melaka's humid rainy season.

受伤当下怎么处理,很大程度决定它好得快不快。这里有一个本地很常见的误区要先纠正——刚扭刚撞,别急着热敷、别马上大力推拿、别猛涂药油「揉散」它。

How you handle a fresh injury largely decides how fast it recovers. One very common local habit needs correcting first — on a fresh twist or knock, don't rush to apply heat, don't have it forcefully massaged, and don't vigorously rub in medicated oil to "break it up."

Cara anda menangani kecederaan baru banyak menentukan kelajuan pemulihan. Satu tabiat tempatan perlu dibetulkan dahulu — pada kecederaan baru, jangan tergesa-gesa panaskan, jangan diurut kuat, dan jangan sapu minyak dengan kuat.

刚受伤时,伤处的小血管正在出血、组织正在肿。热和用力揉会让血管扩张、出血更多、肿得更厉害,反而拖慢恢复。头48–72小时的原则是国际通用的 PRICE:保护(别再碰伤)、相对休息、冰敷(隔着毛巾每次约15–20分钟)、加压(弹性绷带)、抬高(高过心脏)。等这几天过了、肿势稳住,进入恢复期,才由医师判断什么时候适合温通、活血、手法松解。「揉热散瘀」是恢复期的做法,不是急性期的做法——阶段错了,帮倒忙。这也是我们不建议刚受伤就冲去「热热地推开」的原因。

On a fresh injury, the small vessels are bleeding and the tissue is swelling. Heat and hard rubbing widen the vessels, bleed more, and swell worse — slowing recovery. The first 48–72 hours follow the universal PRICE: Protect (no re-injury), Rest (relative), Ice (about 15–20 min through a towel), Compression (elastic bandage), Elevation (above the heart). Once these days pass and the swelling settles into the repair stage, a practitioner judges when warming, blood-moving and hands-on release are appropriate. "Rub it warm to move the bruise" belongs to the repair stage, not the acute one — wrong stage, wrong help.

Pada kecederaan baru, salur kecil berdarah dan tisu bengkak. Panas dan urutan kuat melebarkan salur, lebih berdarah, dan lebih bengkak — melambatkan pemulihan. 48–72 jam pertama ikut PRICE: Lindung, Rehat, Ais (15–20 min, berlapik tuala), Mampatan (pembalut elastik), Tinggikan (atas jantung). 中医处理跌打损伤,关键在「分期」。急性期是「气滞血瘀、肿痛不通」,重在消肿止痛、别扰动;恢复期才「温通活血、散瘀松解」;陈旧伤则「温经通络、化陈瘀」。方向对了,好转体现在三方面——肿和痛的程度减轻、酸痛发作的次数减少、每次不舒服的时间缩短、活动度慢慢恢复。

The key to TCM care for die-da is the stage. A fresh injury is "qi-and-blood stagnation — swollen, painful, blocked," so the priority is to reduce swelling and pain without disturbing it; the repair stage then "warms, moves blood, disperses and releases"; an old injury calls for "warming the channels and clearing long-standing stagnation." Get it right and improvement shows in three ways — less swelling and pain, fewer flare-ups, shorter episodes, and movement gradually returning.

Kunci rawatan TCM untuk die-da ialah peringkatnya. 头两三天以消肿止痛为主。针灸常用「远端取穴」——在离伤处较远的位置下针帮助止痛、促进循环,而不去刺激正在出血肿胀的地方;配合冰敷、加压、抬高让肿先降下来。可按辨证用活血化瘀、消肿止痛的科学中药(浓缩颗粒)内服,或适合的外敷。这阶段绝不在肿处大力推拿或热敷。

The first few days focus on settling swelling and pain. Acupuncture often uses distal points — needling away from the injury to ease pain and aid circulation without provoking the actively bleeding, swollen area — alongside ice, compression and elevation. Depending on the pattern, blood-moving, swelling-reducing herbal granules may be taken internally, or a suitable external application used. No hard massage or heat on the swelling at this stage.

Hari-hari pertama fokus mengurangkan bengkak dan sakit. Akupunktur sering menggunakan titik jauh — mencucuk jauh dari kecederaan untuk melegakan sakit tanpa mengganggu kawasan bengkak — bersama ais, mampatan dan tinggikan. 肿势稳住后,转为帮它「化开、松开、归位」。针灸配艾灸、拔罐温通活血,帮瘀血吸收;轻柔的推拿、理筋松开受伤后紧绷粘连的软组织;若评估合并小关节错位,用正骨手法调回正确位置——往往当场就顺一点、活动度大一点。虚寒瘀滞明显的,配合活血通络的科学中药从内在帮气血运行、筋骨修复。

Once the swelling settles, care shifts to "dispersing, loosening, realigning." Acupuncture with moxibustion and cupping warms and moves blood to help the bruise clear; gentle tuina and sinew work free the tight, adhered soft tissue after the injury; if a small joint is assessed as displaced, a bone-setting adjustment guides it back — often moving a little more smoothly on the spot. For clear cold-stagnation patterns, blood-moving herbal granules support circulation and tissue repair from within.

An old injury never fully cleared — stagnant blood, blocked channels — flares with cold, damp or fatigue. TCM leans on moving blood, freeing the channels and warming out cold, with hands-on work, moxibustion and acupuncture to release the recurring tight knots, then herbs by constitution to cut how often "the weather changes and it aches." Honestly, an old injury is slow work — the goal is lighter aches, fewer flare-ups and shorter episodes, not a one-time wipe-clean.

Kecederaan lama yang tak pernah sembuh — darah tersekat, saluran tersumbat — muncul bila sejuk, lembap atau keletihan. TCM menggerakkan darah, membuka saluran dan memanaskan, dengan kerja tangan, moksibusi dan akupunktur, kemudian herba mengikut perwatakan. Jujurnya, kecederaan lama ialah kerja perlahan.

一条安全底线,这些情况请先看西医、照影像。大部分跌打损伤可以从容按分期调理,但如果出现以下任一种,先去急诊或骨科排除骨折、脱位、韧带完全断裂:受伤当下听到明显「啪」的一声;受伤后完全不能负重、不能走或抬不起来;伤处明显变形、关节看起来「不对位」;剧痛持续不退、很快肿成一大包;伤处远端的手指脚趾发麻、发白、发凉(可能压到血管神经)。头部受伤后出现呕吐、意识模糊、剧烈头痛,要立刻就医、不要等。此外,没有明显外伤史却单个关节红肿热痛,要留意可能是痛风或感染,方向完全不同。确认没有骨折等结构问题后,软组织的瘀肿、酸痛、僵硬,中医可以接手——该照的影像先照,方向清楚了再调,中西医一起看最安全。

One safety line — see a doctor and get imaging first for these. Most die-da can be worked through calmly by stage, but with any of the following, head to A&E or orthopaedics first to rule out a fracture, dislocation or full ligament rupture: a clear "pop" at the moment of injury; being unable to bear weight, walk or lift the part at all; an obviously deformed or "out of place" joint; severe unrelenting pain or fast, huge swelling; toes or fingers beyond the injury going numb, pale or cold (possible vessel or nerve pressure). After a head injury with vomiting, confusion or a severe headache, seek care immediately — do not wait. Also, a single hot, red, swollen joint with no real injury history may be gout or an infection — an entirely different direction. Once structural injury is ruled out, TCM can take over the soft-tissue swelling, ache and stiffness — image first when indicated, then treat with a clear picture; working alongside your doctor is safest.

  • 刚受伤的头两三天,先冰敷、加压、抬高,别热敷别猛揉。用毛巾裹着冰袋敷15–20分钟、用弹性绷带适度加压、把患处垫高过心脏,能有效控制肿胀。这几天别泡热水、别涂药油大力揉、别让人用力推拿伤处——把「揉热散瘀」留到肿退之后由医师判断。真的痛得厉害、肿得快、不能负重,先去照影像。 For the first two or three days, ice, compress and elevate — no heat, no hard rubbing. Ice through a towel for 15–20 minutes, apply moderate compression with an elastic bandage, and raise the part above the heart to control swelling. Avoid hot soaks, vigorous oil-rubbing and forceful massage of the injury these days — leave "rub it warm" until the swelling drops and a practitioner judges. If pain is severe, swelling fast, or you can't bear weight, get imaging first. Dua tiga hari pertama, ais, mampat dan tinggikan — jangan panas, jangan urut kuat. Ais berlapik tuala 15–20 minit, mampat sederhana dengan pembalut elastik, dan tinggikan bahagian itu atas jantung. Elak rendaman panas dan urutan kuat. Jika sakit teruk, bengkak cepat, atau tak boleh menanggung berat, buat imej dahulu.
  • 记下「怎么伤的」和旧伤史。看诊前想清楚:怎么伤的(扭、撞、跌还是拉)、当下有没有听到声音、能不能动、肿和痛在哪、有没有瘀青、这个部位以前伤过没、上次多久才好。陈旧伤反复犯的人,这段病史特别重要——它帮医师判断是新伤、旧患,还是合并了劳损,好安排处理的先后。 Note how it happened and your history. Before the visit, get clear on: how it happened (twist, knock, fall or strain), any sound at the moment, whether you can move it, where the swelling and pain are, any bruising, whether this area was hurt before, and how long last time took. For those with recurring old injuries, this history matters most — it helps the practitioner tell a fresh injury from an old one or an added overuse strain, and sequence the care. Many old injuries come from not letting a fresh one heal, returning to sport or heavy work too soon, so the stagnation never clears and it recurs. After injury, rest relatively and avoid the movements that use the hurt part; in the repair stage, build activity back gradually on the practitioner's advice rather than resuming your full load the moment it stops hurting. Give the body time, and a fresh injury is far less likely to become a lifelong old one. Berhenti dan rehatkan — jangan paksa dengan "nanti okay bila dah panas." 刚扭到、撞到,第一时间该冰敷还是热敷?能不能马上推拿、涂药油揉开?Just twisted or knocked it — ice or heat first? Can I massage it or rub in oil straight away?Baru terseliuh atau terhantuk — ais atau panas dulu? Boleh urut atau sapu minyak terus?+

刚受伤的头48–72小时是「急性期」,要冰敷、不要热敷,也不要用力推拿或猛涂药油去「揉散」它。原因是伤处的小血管正在出血、组织正在肿,热和用力揉会让血管扩张、出血更多、肿得更严重,反而拖慢恢复。急性期的原则是保护、相对休息、冰敷(隔着毛巾约15–20分钟)、加压、抬高。要等肿势稳住、进入恢复期,才由医师判断什么时候适合温通、活血、手法松解。「揉热散瘀」是恢复期的做法,不是急性期的,阶段搞错了会帮倒忙。

The first 48–72 hours after injury is the acute stage: use ice, not heat, and don't massage hard or vigorously rub in medicated oil to 'break it up.' The small vessels are bleeding and the tissue is swelling — heat and hard rubbing widen the vessels, bleed more and swell worse, slowing recovery. The acute rule is Protect, Rest, Ice (about 15–20 min through a towel), Compression, Elevation. Only once the swelling settles and you reach the repair stage does a practitioner judge when warming, blood-moving and hands-on release are right. 'Rub it warm' belongs to the repair stage, not the acute one — wrong stage, wrong help.

48–72 jam pertama selepas cedera ialah peringkat akut: guna ais, bukan panas, dan jangan urut kuat atau sapu minyak dengan kuat. Salur kecil sedang berdarah dan tisu bengkak — panas dan urutan kuat melebarkan salur, lebih berdarah dan lebih bengkak, melambatkan pemulihan. Peraturannya ialah Lindung, Rehat, Ais (15–20 min berlapik tuala), Mampatan, Tinggikan. Hanya selepas bengkak reda dan masuk peringkat pemulihan, barulah pengamal menilai bila sesuai memanaskan dan melepaskan.

跌打损伤看中医怎么处理?要几次才好?How does TCM treat a traumatic injury, and how many sessions does it take?Bagaimana TCM merawat kecederaan die-da, dan berapa sesi diperlukan?+

中医处理跌打损伤讲究分期。急性期(头两三天)以消肿止痛为主,可用活血化瘀、消肿的内服或外敷帮忙,不去扰动伤处;肿退后进入恢复期,用手法松解、针灸、拔罐、艾灸帮助散瘀通络、恢复活动度;陈旧伤则以活血通络、温经为主。需要几次因人而异——看损伤轻重、新旧、有没有合并劳损。好转一般体现在三方面:肿和痛的程度减轻、酸痛发作的次数减少、每次不舒服的时间缩短、活动度慢慢恢复。诚实说:伤得重或拖得久的,恢复需要时间和耐心。

TCM care for die-da is staged. In the acute stage (first few days) the focus is settling swelling and pain, with blood-moving, swelling-reducing herbs taken internally or applied externally, without disturbing the injury; once the swelling drops, the repair stage uses hands-on release, acupuncture, cupping and moxibustion to clear the stagnation and restore movement; an old injury leans on moving blood and warming the channels. How many sessions depends on how severe or old it is and whether there's added strain. Improvement shows in three ways: less swelling and pain, fewer flare-ups, shorter episodes, and movement gradually returning. Honestly, a severe or long-standing injury needs time and patience.

Rawatan TCM untuk die-da mengikut peringkat. Waktu akut (hari-hari pertama) fokus mengurangkan bengkak dan sakit, dengan herba yang menggerakkan darah, tanpa mengganggu kecederaan; selepas bengkak turun, peringkat pemulihan menggunakan pelepasan tangan, akupunktur, bekam dan moksibusi untuk menyerak sekatan dan memulihkan pergerakan; kecederaan lama bergantung pada menggerakkan darah dan memanaskan saluran. Bilangan sesi bergantung pada keterukan dan usia kecederaan. Pemulihan ditunjukkan melalui: kurang bengkak dan sakit, kurang kerap muncul, tempoh lebih pendek. Jujurnya, kecederaan teruk atau lama perlukan masa dan kesabaran.

旧伤一变天、一劳累就酸痛,还能调吗?An old injury aches whenever the weather changes or I'm tired — can it still be treated?Kecederaan lama sakit bila cuaca berubah atau penat — masih boleh dirawat?+

可以。很多「陈旧伤」是当年受伤时没好好处理,瘀血没散干净、筋络没完全修复,留下反复酸痛僵硬,遇冷、遇潮、劳累就犯。中医看这类属陈旧瘀阻、经络不通,常夹寒湿,调理方向是活血通络、温经散寒、松解粘连的软组织,配合科学中药(浓缩颗粒)从内在帮气血运行。诚实说:陈旧伤调理是慢工,目标是让酸痛程度更轻、犯的次数更少、每次更短,而不是一次清零。马六甲雨季潮湿,这类旧患特别容易犯。

Yes. Many 'old injuries' were never properly treated at the time — stagnant blood never fully cleared and the sinews never fully healed — leaving recurring aches and stiffness that flare with cold, damp or fatigue. TCM sees this as long-standing stagnation and blocked channels, often with cold-damp, and treats it by moving blood, freeing the channels, warming out cold and releasing the adhered soft tissue, with concentrated herbal granules supporting circulation from within. Honestly, it's slow work — the goal is lighter aches, fewer flare-ups and shorter episodes, not a one-time wipe-clean. Melaka's humid rainy season makes these old injuries flare especially easily.

Boleh. Banyak 'kecederaan lama' tak pernah dirawat dengan betul — darah tersekat tak hilang sepenuhnya dan urat tak sembuh penuh — meninggalkan sakit dan kekakuan berulang yang muncul bila sejuk, lembap atau penat. TCM melihatnya sebagai sekatan lama dan saluran tersumbat, selalunya dengan sejuk-lembap, dan merawatnya dengan menggerakkan darah, membuka saluran, memanaskan dan melepaskan tisu lembut, dengan granul herba menyokong peredaran. Jujurnya, ia kerja perlahan. Musim hujan lembap Melaka menyebabkan kecederaan lama ini mudah muncul.

什么情况要先去照X光或看西医,不能只看中医?When should I get an X-ray or see a doctor first, rather than only TCM?Bila perlu buat X-ray atau jumpa doktor dahulu, bukan hanya TCM?+

出现以下任一种,先去急诊或骨科排除骨折、脱位、韧带完全断裂:受伤当下听到明显「啪」的一声;受伤后完全不能负重、不能走或抬不起来;伤处明显变形、关节看起来「不对位」;剧痛持续不退、很快肿成一大包;伤处远端的手指脚趾发麻、发白、发凉(可能压到血管神经)。头部受伤后出现呕吐、意识模糊、剧烈头痛,要立刻就医、不要等。确认没有骨折等结构问题后,软组织的瘀肿、酸痛、僵硬中医可以接手——该照的影像先照,方向清楚了再调,中西医一起看最安全。

With any of the following, go to A&E or orthopaedics first to rule out a fracture, dislocation or full ligament rupture: a clear 'pop' at the moment of injury; being unable to bear weight, walk or lift the part at all; an obviously deformed or 'out of place' joint; severe unrelenting pain or fast, huge swelling; toes or fingers beyond the injury going numb, pale or cold (possible vessel or nerve pressure). After a head injury with vomiting, confusion or a severe headache, seek care immediately — don't wait. Once structural injury is ruled out, TCM can take over the soft-tissue swelling, ache and stiffness — image first when indicated, then treat with a clear picture; working alongside your doctor is safest.

Dengan mana-mana berikut, ke kecemasan atau ortopedik dahulu untuk menolak patah, tergelincir atau ligamen koyak penuh: bunyi 'pop' jelas semasa cedera; tak boleh menanggung berat, berjalan atau mengangkat langsung; sendi nampak herot atau 'tak kena tempat'; sakit teruk berterusan atau bengkak besar cepat; jari atau kaki di luar kecederaan kebas, pucat atau sejuk. Selepas kecederaan kepala dengan muntah, keliru atau sakit kepala teruk, dapatkan rawatan segera. Selepas kecederaan struktur ditolak, TCM boleh mengambil alih bengkak dan sakit tisu lembut — buat imej dahulu bila perlu.

「跌打」和「正骨」是一样的吗?Is 'die-da' the same as bone-setting?Adakah 'die-da' sama dengan urut tulang?+

不完全一样。「跌打损伤」是一类伤——指外力(扭、撞、跌、挫、拉)造成的筋肉和软组织急性损伤,以及没处理好留下的陈旧伤,核心是气滞血瘀、伤筋动络。「正骨」是一种手法——处理的是关节错位、小关节卡住、体态结构的问题。两者会有交集(比如用力一崴,既有软组织跌打伤,也可能有小关节错位需要正骨调回去),但不是同一回事。另外,「跌打」比「单一关节扭伤」范围更广——脚踝扭伤只是其中一种,手腕、肩、腰、膝、肌肉拉伤都在里面。到诊后医师会辨证判断你这一次以哪一类为主、怎么搭配处理。

Not quite. 'Die-da' (traumatic injury) is a category of injury — acute soft-tissue and sinew injury from external force (twists, knocks, falls, bruises, strains), plus old injuries that were never properly treated; at its core it's qi-and-blood stagnation with injured sinews and channels. 'Bone-setting' is a technique — it addresses a jammed or displaced joint and postural structure. They overlap (a hard roll may carry both a soft-tissue injury and a slightly displaced joint that bone-setting guides back), but they're not the same thing. And 'die-da' is broader than 'a sprained ankle' — the ankle is just one; wrists, shoulders, lower back, knees and muscle strains all belong. At the visit the practitioner works out which predominates and how to combine the care.

Tidak sepenuhnya. 'Die-da' (kecederaan) ialah kategori kecederaan — kecederaan akut tisu lembut dan urat akibat daya luar (pusing, hentam, jatuh, lebam, tarik), serta kecederaan lama yang tak dirawat; terasnya qi dan darah tersekat dengan urat cedera. 'Urut tulang' ialah teknik — menangani sendi tersekat atau teranjak dan struktur postur. Keduanya bertindih, tetapi bukan perkara yang sama. Dan 'die-da' lebih luas daripada 'terseliuh buku lali' — buku lali hanya satu; pergelangan tangan, bahu, pinggang, lutut dan otot tertarik semua termasuk.
