很多家庭的餐桌像战场:孩子含着一口饭半天不吞、追着喂满屋跑、这不吃那不吃,好不容易塞几口,一会儿又说肚子胀、不舒服。零食饮料倒是来者不拒,正餐永远没胃口。看着孩子瘦瘦小小、脸色黄黄、动不动喊累,家长又急又累:「是不是发育不好?要不要买点开胃的、补的?」先别急着「催」或「补」。中医的看法是:孩子的问题多半出在「脾胃」——这个负责消化吸收、把食物变成气血的「后天之本」还很娇嫩。脾胃没调好,硬催硬补只会更糟;把脾胃理顺了,孩子自然会「愿意吃、吃了能长」。
For many families the dinner table is a battlefield: a mouthful of rice held un-swallowed for ages, chasing the child round the house, "won't eat this, won't eat that," and once a few bites are in, soon a bloated, uncomfortable tummy. Snacks and drinks, though, are never refused; it's proper meals that meet no appetite. Watching a small, thin, sallow, easily-tired child, parents grow anxious and worn: "Is the development off? Should I buy something to boost appetite, something tonic?" Don't rush to "force" or "tonify." TCM's view: the trouble is usually in the "spleen and stomach" — the "acquired root" that digests, absorbs, and turns food into qi and blood, still tender in a child. With the spleen untended, forcing and tonifying only make it worse; set the spleen right, and a child will naturally "want to eat, and grow from it."

孩子长期食欲差、吃一点就饱、老是积食,西医多从「喂养行为」「消化功能」来看。中医看的是背后的核心——脾胃虚弱、乳食积滞。中医说「脾常不足」是孩子的生理特点:脾胃是把食物转化成气血、供养全身的「后天之本」,孩子还在发育、这套系统本就娇嫩。若脾胃虚,动力不够,就「不想吃、吃了不消化」;食物堆在肠胃化不掉,就成「积食」,胀气、口臭、大便不调、睡不安、还容易一积就发烧生病。虚(脾胃弱)和实(积食)常常交织在一起:越虚越容易积,越积又越伤脾,形成恶性循环。所以中医不是一味「催食」或「猛补」,而是先把脾胃的运化功能理顺。
A child's long-standing poor appetite, quick fullness and frequent food stagnation are usually viewed through "feeding behaviour" and "digestion" in Western medicine. TCM looks at the core behind them — spleen-and-stomach weakness with retained food. Children are "constitutionally spleen-weak": the spleen and stomach are the "acquired root" that turns food into qi and blood, still tender in a growing child. When the spleen is weak and lacks drive, the child "won't eat, and doesn't digest what's eaten"; food piles up undigested into "stagnation" — bloating, bad breath, irregular stools, restless sleep, and a tendency to spike a fever or fall ill once stagnant. Deficiency (weak spleen) and excess (stagnation) usually interweave: the weaker, the easier to stagnate; the more stagnation, the more it harms the spleen — a vicious cycle. So TCM doesn't just "push food" or "pile on tonics" but first sets the spleen's transporting function right.
Selera lemah berpanjangan, cepat kenyang dan kerap sembelit makanan pada anak biasanya dilihat melalui "tingkah laku pemakanan." TCM melihat terasnya — kelemahan limpa-perut dengan makanan tertahan. Poor appetite and easy stagnation are mostly chronic states that can be worked through calmly; but some "won't grow" or "won't eat" hides a problem to check first. With any of the following, see a paediatrician first: weight not gaining or falling, or height and weight clearly behind peers / dropping off the growth curve; persistent vomiting, recurrent diarrhoea or constipation, or blood in the stool; persistent or severe abdominal pain, a hard, drum-tight belly; difficulty swallowing, choking or vomiting on eating; food refusal with a very listless, visibly wasted or dehydrated child; clear delay in development, speech or activity. These can point to allergy, infection, or a digestive or endocrine problem to clarify first. TCM spleen care never conflicts with medical assessment — we'll advise referral honestly when it's needed.

The deepest layer. TCM says children are "constitutionally spleen-weak" — the digestive system is still developing and its transporting power is naturally low. A frail constitution, prematurity, or weaning that added foods too fast and too varied leaves the spleen thinner still. Common signs: persistently poor appetite, full after a little, a sallow face, thin build, stools that swing dry and loose, a white or thick tongue coating. The spleen is the "acquired root"; a thin one can't transform what's eaten, so the child doesn't grow on it — the layer TCM most wants to set right.
Lapisan paling dalam. TCM mengatakan anak "lemah limpa secara semula jadi" — sistem penghadaman masih membesar. The most changeable link, and the most overlooked. A stream of snacks, biscuits, sweets, sugary drinks and ice cream fills the tummy well before mealtimes, so of course there's no appetite; raw-cold and cloying foods hit the spleen hardest. Chasing to feed, eating while playing or watching a screen, no set mealtimes, and overeating in one sitting all throw the spleen's rhythm off. Many children aren't "unable to eat" — these habits have unsettled the appetite and the spleen — setting feeding right often matters more than any medicine.

很多孩子的胃口是「一场病之后」变差的。反复感冒、发烧、腹泻,本身就耗伤脾胃之气;而退烧药、抗生素等(该用时要用)多偏寒凉,一轮轮下来,脾胃更虚更冷,孩子病刚好那几天往往特别不想吃。若这时家长心疼、急着「补一补」大鱼大肉,反而加重积食。脾胃虚 → 容易积食生病 → 生病用药又伤脾,正是很多孩子陷进去的循环。中医常在病后帮孩子「醒脾开胃」,把这一环接上。
Many children's appetite drops "after an illness." Recurrent colds, fevers and diarrhoea drain the spleen-stomach qi; and fever meds and antibiotics (use them when needed) tend to be cooling, so round after round leaves the spleen weaker and colder — a child often eats especially poorly in the days just after a cold. If a worried parent rushes a rich "let's build him up" feast then, it worsens stagnation. Weak spleen → easy stagnation and illness → illness and medicine tire the spleen again — the very loop many children fall into. TCM often helps "wake the spleen and open the appetite" after illness to break it.
Selera ramai anak jatuh "selepas sakit." Selesema, demam dan cirit-birit berulang menyusutkan qi limpa-perut; dan ubat demam serta antibiotik (guna bila perlu) cenderung menyejukkan, jadi pusingan demi pusingan meninggalkan limpa lebih lemah. Jika ibu bapa tergesa jamuan berat ketika itu, ia memburukkan sembelit. Limpa lemah → mudah sembelit dan sakit → sakit dan ubat melemahkan limpa lagi. When mealtimes become a tug-of-war, emotion enters. Chasing, forcing, scolding over unfinished food ties "eating" to "unpleasant," so the child resists more as mealtime nears; TCM also holds that "over-thinking harms the spleen" and "liver qi invades the spleen" — a tense, stressed child, or a tense household, and the spleen "goes on strike" too. Sometimes the problem isn't the child's stomach but the mood at the table. Easing the pressure and letting meals be pleasant again is an important, often-overlooked part of care.

Care for a child centres on "building the spleen, resolving stagnation, opening the appetite": clearing the stuck food first, then rebuilding the weak spleen-stomach, while helping parents set feeding right. Methods stay gentle and easy to accept — led by what a child will cooperate with, not centred on needling, and never "force-fattening." Get it right and parents usually come to feel the child willing to eat, less stagnation, smoother stools, better energy and colour, with weight and height following their own track. It takes a little patience and partnership in feeding.

By the child's tongue, belly and stools, the practitioner prescribes concentrated herbal granules — for marked stagnation, first food-resolving herbs like hawthorn, medicated leaven and malt; for marked deficiency, spleen-strengthening formulas; for mixed pictures, resolving and tonifying together. Granules are dosed to the child's weight, dissolve in water, and taste far milder than brewed decoctions, so most children accept them. The logic is "clear first, then build; resolve and nourish together," so the spleen turns over on its own — not forcing food with an "appetite drug." All prescribing is by a KKM-registered practitioner — don't buy online "children's appetite / weight-gain" products yourself.
Tunjang untuk limpa. Mengikut lidah, perut dan najis anak, pengamal memberi granul herba tumpat — untuk sembelit ketara, herba pelerai makanan seperti hawthorn dan malt; untuk kelemahan ketara, formula penguat limpa. Granul didos ikut berat anak, larut dalam air, lembut rasanya. Logiknya "bersihkan dahulu, kemudian bina." Semua preskripsi oleh pengamal berdaftar KKM — jangan beli produk "penambah selera / berat" dalam talian sendiri.

A gentle route for children wary of medicine or needles. Acupoint plasters place spleen-strengthening, food-resolving, warming herbs on relevant points on the abdomen and back (around Zhongwan, Shenque, Pishu) — nothing to swallow, nothing to needle — helping wake the spleen, open the appetite, and ease bloating and stagnation. Gentle moxibustion warms the spleen-stomach and dispels cold in the middle. For a spleen-cold child who loosens on cold food or has a cool tummy, these gentle external methods suit especially well. Which points, how long, and whether to use moxa are set by the practitioner to the child's age and constitution — parents shouldn't moxa or plaster a child's belly on their own.

This link often matters more than medicine — because the spleen is "eaten" into shape every day. The practitioner helps parents reset feeding: set mealtimes and amounts, no chasing or forcing, no snacks or drinks before meals, fewer raw-cold and cloying foods, eat focused without a screen, stop at comfortably-full; in the weak days after illness, keep food light and easy first, don't rush tonics. Clinic herbs and plasters "lift the spleen up"; home feeding "keeps it from being unsettled again" — together, the child truly enters the "wants to eat, grows from it" cycle. Eating with your child every day, you are the most important person in this care.

Notes for parents. ① Don't force-feed, chase, or use food as reward/punishment. Forcing makes a child resist eating more, and a tense mood shuts the spleen down further — it backfires. ② Don't buy "appetite drugs / weight-gain powders / children's supplements" and give them freely. Unclear contents, or things a child shouldn't have, can harm the spleen and body; "tonifying" needs pattern diagnosis — forcing tonics on stagnation or internal heat only blocks more. Have all herbs prescribed by a KKM-registered practitioner who has seen the child. ③ Don't cut down an adult's medicine or tonic for a child yourself. ④ Work with Western medicine: check what needs checking first (see "🚩" above) and use medicine where needed; TCM tends the spleen baseline. Tell each practitioner what the child takes and any allergies.

A relaxed, pleasant table is what makes the spleen "willing to work." They may eat little at first — don't panic; a little hunger brings the appetite back better than forcing. Iced water, cold drinks and lots of raw-cold foods hit a tender spleen hard — swap to warm water and room-temperature or warm foods. Biskut, gula-gula, minuman manis, aiskrim dan gorengan ialah beban utama limpa — simpan kurang, beri kurang, dan tiada snek satu dua jam sebelum makan. Air ais dan makanan sejuk-mentah membebankan limpa lembut — tukar kepada air suam dan makanan suhu bilik. Masakan lembut lebih sesuai.

Before the visit, observe: how is the appetite through the day? How much is eaten? Any bloating, bad breath, dry or loose stools, restless sleep, thick tongue coating? Thin, or puffy-weak? Describing these helps the practitioner tell "deficiency" from "stagnation." In the days just after an illness the spleen is weakest — keep food light first, don't rush rich tonics and meat. Give the child moderate daily activity — the spleen "transforms better with movement," and it helps growth too. Don't let thinness push you to force food — go with the spleen's rhythm and it holds.
